The Status Of The Research On Application Of The Chinese Guidelines For The Management Of Hypertension In Community Chronic Diseases Health Management

Objective:Hypertension is one of the most common cardiovascular diseases, with the rapid development of the economy and the way of life changed, the incidence of hypertension showed a trend of rise. At present the China hypertension prevention and cure guide in grass-roots community health service center also has a certain practical difficulty of application of the awareness, treatment and control of high blood pressure remains to be improved.Through this research, aimed at using scientific research methods, prevention and control of Chinese guidelines for the management of hypertension in grassroots medical institutions especially the application of urban community health service center of community chronic disease management situation, puts forward effective feasible countermeasures and Suggestions, so as to effectively improve grassroots awareness, treatment and control of high blood pressure, maximum limit reduces the risk of cardiovascular morbidity and mortality.Methods:Through sampling method (questionnaire survey, project discussion) survey 10 community health service centers of the Xuanwu borough in guidelines for the management of hypertension of chronic diseases management.Survey content includes to the medical personnel engaged in chronic disease health management questionnaire investigation, In the Xinjiekou?Suojincun?Xianhemen three community health service center part of the questionnaire survey to hypertensive patients, By means of focus group discussion and a major leaders in discussion of 10 community health service centers of the Xuanwu borough, Chronic disease health management of guidelines for the management of hypertension in the Xuanwu borough community to discuss the application status investigation. Investigation reference parameters of the 2010 Chinese guidelines for the management of hypertension, the Chinese guidelines for the grass-roots management of hypertension(Revised Edition 2014).The survey data using SPSS 17.0 statistical software for data analysis.Results:According to the design, handed out 920 questionnaires, this study obtained 858 the effective questionnaire (93.3%), Including 63 chronic disease managemen of high blood pressure, from the medical staff of community in 10 community health service centers of the Xuanwu borough. And 795 effective questionnaire of hypertension patients in community, from 11 community in 3 community health service centers of the Xuanwu borough.The medical staff of different cultural degree?different fixed number of year of the hypertension health management to guide how familiar are statistically significant (P< 0.05); And the medical staff of different sex, different titles to guide know no statistical difference (P> 0.05). The high education level of personnel and personnel engaged in long duration of hypertension disease management, more willing to accept the training and guidance on the guidelines.Degree of different culture, different regions of hypertension patients in community to accept the situation of the community hypertension management no statistical difference (P> 0.05); Different knowing life of the community hypertension patients accept community hypertension management is statistically significant (P< 0.05).Degree of different culture, different regions of hypertension patients in community and willing to accept the community hypertension management has no obvious difference, knows hypertension duration of the longer the patients more willing to accept the community hypertension management.Community management personnel and hypertension patients in community patients on chronic diseases to the superior for the statistical analysis results show that the major cause of hospital during chronic disease management advice, mistrust of the community, or to the big hospital insurance, both to the superior big hospital cause statistically significant (P< 0.05); While the community clinic accepts doctor advice, by feeling, both to the superior big hospital because no statistical difference (P> 0.05).For the purpose of this study, chronic disease community management using the main factors that influence the prevention and control of hypertension guidelines is cultural degree and degree of understanding, the secondary influence factors is to manage the fixed number of year, training; Community hypertension researchers accept chronic hypertension community management main influence factor is the cultural level and hypertension management method, fixed number of year of the secondary influence factors is to accept management, increase the degree of knowledge, understand the necessity of health teaching materials.Fixed number of year of the degree of chronic disease community management culture, management, training, extent of understanding, using the intervention, the promotion of the evaluation of the six factors (P< 0.05) and chronic disease community management staff that there is a connection using the prevention and control of hypertension guidelines, introducing the unconditioned logistic regression analysis model for multiple factors, by hypertension prevention and treatment guidelines were chronic disease community management there are statistically significant factors are:the degree of culture, knowledge and use of intervention. For the purpose of this study, chronic disease community management using the main influencing factors of prevention and control of hypertension guidelines is the degree of culture, knowledge, use intervention, fixed number of year of the secondary influence factors are management, training, promotion of evaluation.Fixed number of year of the degree of hypertension patients in community culture, accept management, high blood pressure management methods, improve the degree of knowledge, understand the necessity of health teaching materials five factors (P< 0.05) with the community hypertension patients accept a link between chronic hypertension community management, introducing the unconditioned logistic regression analysis model for multiple factors, affect the community hypertension patients accept hypertension community chronic disease management there are statistically significant factors are:cultural level, high blood pressure, management methods and understand the health degree of teaching materials. For the purpose of this study, the community hypertension patients receiving chronic hypertension community management is the main factors affecting the degree of culture, high blood pressure management methods and understand the health degree of teaching materials, the secondary influencing factors is to accept the necessity of management fixed number of year, improve knowledge.Conclusions:1. Hypertension patients in community need to apply the Chinese guidelines for the management of hypertension for chronic disease management;2. Chronic disease management professional literacy and skills on the low side, lack of initiative affect their application guidelines for management;3. Low culture degree and the ability to understand of hypertension patients in community, the non contract management mainly affect their acceptance of management;4.
