| Objective:To compare the difference on clinicopathological characteristics between proximal and distal colon carcinoma,in order to provide scientific evidence for clinical diagnosis and treatment of different location of colon cancer.Method: Liaoning Tumor Hospital from January 2010 to May 2011 and postoperative pathology confirmed the diagnosis of colon cancer with complete clinical and pathological data of 457 patients were retrospectively analyzed.The boundary of colon cancer is the splenic flexure of colon that divided colon patients into left and right colon cancer.The former includes of splenic flexure of colon,descending colon,sigmoid colon,and the latter includes the ileocecal valve,ascending colon,hepatic flexure of the colon and transverse colon..All the data were in inputted into compute by using Epidata3.0.We collected patients data on gender,age,initial symptoms,pathological type,Dukes installments,blood type distribution,blood platelet count and so on.Statistical analysis were conducted by using SPSS 19.0 Statistical Soft Package.Ratio used for descriptive analysis.Analysis of categorical data was performed by Chi-square test.Results:1,Among the 457 cases which were analysised,right-sided and left-sided were 242and215 cases respectively,and average were 61.99.The youngest patient was 23 years old,and the oldest was 90 years old.<40years old were 26 cases(including 8 cases of left colon cancer,and 18 cases of right colon cancer);40~60 years old were 188 cases(including 92 cases of left colon cancer,and 96 cases of right colon cancer);>60 years old were 243 cases(including 115 cases of left colon cancer,and 128 cases of right colon cancer).The age of the right-and left-sided colon cancer groups was no statistical difference(P>0.05). 2,In the left-sided colon cancer group,the ratio of male to female is 55.8% vs 44.2%;in the right-sided colon cancer group,the ratio of male to female is 47.9% vs 52.1%.Compared with the left colon,right colon cancer was more common in female patients,but the difference was not statistically significant(P > 0.05)3,The initial symptoms of right-sided colon cancer were common in abdominal pain and abdominal distension or abdominal discomfort;while left-sided colon cancer often with hematochezia or tarry stool for the first symptom.The difference was statistically significant(P<0.05).4,The ratio of Dukes D period was higher in right-sided colon cancer than left-sided colon cancer(P<0.05).5,In gross tumor shape,infiltration ulcerative in left and right colon cancer are common,the difference was not statistically significant(P > 0.05).6,The Pathological type of the cases with colon cancer,and moderately differentiated adenocarinoma were common in left-sided colon cancer;the ratio of moderately and poorly differentiated adenocarinoma,mucinous adenocarcinoma /signet ring cell carcinoma,poorly differentiated adenocarinoma were higher in right-sided colon cancer than left-sided colon cancer.The difference was statistically significant(P<0.05).7,There were 51 patients with high-platelet in left colon cancer,and 119 cases in right colon cancer.The difference was statistically significant(P<0.05).8,Left and right colon cancer in APTT existed significant difference,but both mean were within the normal reference range,and FIB mean greater than normal value,but with no statistical significance(P > 0.05).9,The ABO blood group distribution in left half colon cancer was B > A > O > AB;the right colon cancer was O > A > B > AB,two groups in the distribution of blood group was no significant difference(P > 0.05).Conclusion:1,The initial symptoms of right-sided colon cancer were common in abdominal pain and abdominal distension or abdominal discomfort.Left-sided colon cancer were commom in hematochezia or tarry stool for the first symptom.2,Compared with the left colon,differentiation of the right colon cancer was worse.And patients with advanced were more common in the right colon cancer.3,Platelet abnormal incidence in the right colon cancer was significantly higher than that of the left colon,and increased more. |
