Clinical Analysis Of Syndrome Pattern Of TCM For Distributing Type Vitiligo

Objective: To study the feature and distribution of syndrome pattern of traditional Chinese medicine distribution(TCM)for distributing type vitiligo,analyse the relationship of TCM syndrome pattern with the course of disease,stage of disease,and age of patients with statistical analysis,so as to guide the syndrome differentiation of traditional Chinese medicine and clinical medical care.Methods: The 108 cases were collected during January 2015 to December 2015 in our department during outpatient clinic diagnosis for patients with vitiligo data.Workers fill in the questionnaire,who were divided to 5 types:Wind-heat-dampness syndrome,deficiency of the liver and kidney,depression of liver-qi,blood stasis in collaterals and weakness of the spleen and stomach syndrome,according to the Chinese society of combining traditional Chinese and western medicine skin disease epidemiology group of professional committee of pigment of 2009 patients with vitiligo treatment consensus.And those who did not clear dialectical type are confirmed as unknown groups.Statistical analysis of 108 cases of patients with vitiligo of traditional Chinese medicine and its relationship with age,course of disease,clinical staging.Results: 1.The type of syndrome in 108 cases,wind-heat-dampness syndrome has20 cases(18.5%),deficiency of the liver and kidney type has 26 cases(24.1%),depression of liver—qi type 26 cases(24.1%),spleen and stomach weak type 18 cases(16.4%),and blood stasis resistance winding type 4 cases(3.6%),unclear type 14 cases(12.7%).2.The different age stages of TCM syndrome types in general have difference(p <0.05);the proportion of 16 to 35 age group is the most,accounting for 50%;< 16 years old age group majority has weakness of spleen and stomach,accounting for 32.1%;16and 35 years old age group chiefly belongs to deficiency of the liver and kidney type,accounting for 29.6%;> 35 years of age with liver depression and qi stagnation occupies the larger proportion,accounting for 42.3%.(P < 0.05)3.The ratio of patients in stable is more than in progression,were 65 cases(60.2%),43patients(39.8%),there are differences between the different stages of TCM syndrome types(P < 0.05).Occupied a higher proportion of patients with wind-heat-ampness syndrome,28.9%,and the stability of vitiligo is given priority to with liver and kidney deficiency type,accounting for 30.8%,patients with kidney deficiency type,more stability than progress period(P < o.05);Progress phase is given priority to with rheumatism wind-cold,accounted for 30.2%,tip more wind-heat-ampness syndrome(P<(0.05).there exits significant difference(P < 0.05)4.The proportion of patients whose course of disease under 1 year chiefly belongs to wind-heat-ampness syndrome accounts for 23.4% liver and kidney deficiency accounts for 25%.The course of disease with more than 1 year mainly liver depression and qi type,accounted for 34.1%,with statistical significance(P < 0.05);there are changes in the proportion of TCM syndrome types if it was divided into two type :course of disease in one year or less than 1 year,& more than 1year.wind—heat—dampness syndrome ratio decreased,acccounting 23.4%,11.45 respectively,liver depression and qi stagnation type proportional rise,acccounting for 17.2% & 22.3% respectively(P <0.05);Liver and kidney deficiency in the course of one year or less,> 1 year proportion is higher,25%,22.7% respectively(P < 0.05);Under uncertainty in one year or less than > 1 year group,15.6%,9% respectively.Conclusion:There are certain difference and regularity in the TCM syndrome types of patients with distributing type vitiligo in the course of disease,stage of disease,and age of patients,so individualized treatmen is necessary when we attend patients,specific as follows:1.Show the disease in liver and kidney deficiency type,liver depression and qi as themain syndrome types.2.The age of onset stage,on the basis of dispersing wind—heat—dampness,children patients with spleen and stomach weak type is prior,should strengthen the spleen and lung;Young people should pay attention to tonic liver and kidney;Middle-aged focus on dispersing liver depression and qi,and often keep good mood.3.On the clinical stage,progress period should with functions of moisture,harmonize qi and blood to the plateau appropriate is given priority to with tonic liver and kidney,harmonize qi and blood,and liver resolve depression,promoting blood circulation to remove blood stasis should take into account.4.In the course of the disease,for short course of disease,treatment should be given when notice breeze and moisture;When focusing on the course of elderly liver resolve depression,invigorate the circulation of wind,and the necessary psychological counseling treatment,the adjusting of modern;Nourishing liver and kidney should be throughout the course of the disease.
