Objective: To compare the difference of the incidence of geriatric syndrome between different glucose metabolism and relative factors in 180 elderly inpatients,in order to investigate the prevalence of geriatric syndrome in the elderly inpatients with different glucose metabolism and its influencing factors.Methods: To collect the clinical data of 180 cases elderly inpatients at an age of over65 years from geriatric department within the second hospital of Dalian during November 2014 to November 2015.According to the gender,age(65~69 years,70~74years,75 years~),education(primary school,middle school,high school,junior college,bachelor degree or above),living arrangements(living alone,living with relatives,)and whether diabetes mellitus(diabetes mellitus,non-diabetes mellitus),those patients were divided into different groups.Including visual loss,hearing loss,malnutrition risk,urinary incontinence,chronic constipation,chronic pain,multiple co-morbidity,polypharmacy,impairment in activities of daily living,high risk of falling,cognitive impairment and emotional disorder,to compare the difference of the incidence of geriatric syndrome,medications and comorbidities between different groups.According to the course of diabetes(course<10 years,course≥10 years),Hb A1 c level(Hb A1c≤7.5%,Hb A1 c > 7.5%)and the number of diabetes-related chronic complications(no diabetes-related chronic complication,one diabetes-related chronic complication,more than two diabetes-related chronic complications),111 elderly patients with diabetes mellitus were divided into different groups,to compare the difference of the incidence of geriatric syndrome,medications and comorbidities between different groups respectively.Results: 1.Among 180 elderly patients were with 2 to 20 types of comorbidities,they averagely had(9.71±3.40)comorbidities and(8.84±3.84)medications.The incidence of visual loss,multiple co-morbidity and polypharmacy was higher at 86.11%、85.56%and 82.78%.Secondly,the incidence of impairment in activities of daily living,urinary incontinence,chronic constipation and chronic pain was 56.11%、49.44%、46.67% and42.22%,respectively.2.Among 180 elderly patients,there were 83 males and 97 females,the ratio of male and female was 0.856:1.When compared with men,women were found to have higher incidence of geriatric syndrome such as urinary incontinence,chronic pain,impairment in activities of daily living and high risk of falling(P <0.05).3.180 elderly patients were divided into different groups according to age,with the age increasing,the incidence of geriatric syndrome such as hearing loss and chronic constipation,as well as medications were accordingly higher(P <0.05).4.With the education increasing,the incidence of geriatric syndrome such as urinary incontinence and impairment in activities of daily living was generally declined(P <0.05).5.Living alone group of 22 elderly patients,accounting for 12.22% of the total.There were no difference in the incidence of geriatric syndrome,comorbidities and medications between living alone group and living with relatives group.6.Comparing with non-diabetes mellitus group,waist-hip ratio,comorbidities,medications and the incidence of geriatric syndrome such as multiple co-morbidity,polypharmacy and impairment in activities of daily living of diabetes mellitus group were higher(P <0.05).7.111 elderly patients with diabetes mellitus were divided into course<10 years group and course≥10 years group according to the course of diabetes.The incidence of geriatric syndrome such as visual loss and polypharmacy,as well as medications were higher in course≥10 years group(P <0.05).8.Comparing with Hb A1c≤7.5% group,the incidence of geriatric syndrome suchas visual loss and urinary incontinence in Hb A1c>7.5% group were higher(P <0.05).9.With the numbers of diabetes-related chronic complications increasing,the incidence of geriatric syndrome such as visual loss,chronic constipation and multiple co-morbidity,as well as comorbidities and medications were higher.Comparing with no diabetes-related chronic complication,the incidence of visual loss and comorbidities was higher in between one diabetes-related chronic complication group and more than two diabetes-related chronic complications group(P <0.05).Conclusions: 1.Elderly patients generally suffer from geriatric syndrome such as visual loss,urinary incontinence,chronic constipation,chronic pain,multiple co-morbidity,polypharmacy and impairment in activities of daily living,as well as comorbidities and medications are higher.2.Elderly women are much easier to suffer from urinary incontinence,chronic pain,impairment in activities of daily living and high risk of falling.With the age increasing,the incidence of the hearing loss and chronic constipation,as well as medications are accordingly higher.With the education increasing,the incidence of urinary incontinence and impairment in activities of daily living is generally decline.3.Elderly patients with diabetes mellitus are much easier to suffer from geriatric syndrome such as multiple co-morbidity,polypharmacy and impairment in activities of daily living respectively.The incidence of the visual loss and polypharmacy,as well as medications are higher in course≥10 years group.The incidence of the visual loss and urinary incontinence in Hb A1 c > 7.5% group is higher.With the numbers of diabetes-related chronic complications increasing,the incidence of the visual loss,chronic constipation and multiple co-morbidity,as well as comorbidities and medications are higher. |
