Background:Diabetes, malignant tumor, cardiovascular disease have become three severe threat to human health;in recent years, epidemiological data shows that diabetes and cancer incidence frequently increased year by year, evidently as two significant chronic diseases, serious threat to human health, the national economic burden becomes heavy;the relationship between diabetes and malignant tumor has become a hot spot, In hospitals, some characteristics found or prognosed between type 2 diabetes mellitus and malignant tumor have significant resemblance. Research in type 2 diabetes mellitus merging malignant tumor clinical characteristics, and provide more abundant clinical basis to future treatment. Objective:To investigate the clinical characteristics of patients with type 2 diabetes mellitus; and to study the risk factors; and to provide some clinical evidence for early diagnosis,early prevention and complicated treatment of diabetes mellitus with malignant tumor. Methods:Continuous selection from March 2012 to March 2015 Nanchang University Second Affiliated Hospital 8087 cases of inpatient with type 2 diabetes mellitus, divided into simple diabetic group(SDG) and malignant tumor group(MTG) merged based on whether complicated with malignant tumor, collecting clinical data, assessing malignant tumor in type 2 diabetic patients with clinical features and risk factors for statistical analysis. Result:1.Hospitalized patients with type 2 diabetes, detection rate of malignant tumor is 5.7%, the top five male with cancer cases are: colorectal(23.49%), lung(15.10%), liver(14.43%), gastric(9.73%), and bladder(5.37%), and top five female patients respectively: colorectal cancer(20.12%), breast cancer(9.76%), endometrial carcinoma, gastric cancer(9.15%), lung cancer(7.32%), liver, gallbladder cancer(6.10%).2.Course of high prevalence for more than 20 years of malignancy(P = 0.002), the peak onset age is at the age of 60-80,which is accounted for 59.7%, males have more risk for malignant tumor(P<0.001), type 2 diabetes of the top five malignant tumor is as follows: colorectal(22.29%), lung(12.34%), liver(11.47%), gastric(9.52%),Bladder(4.76%), which is proportion of 60.38% in all patients with malignant tumor, digestive system tumors largest percentage(51.95%).3.The result of logistic stepwise regression analysis indicates the OR(95% CI) are: age 1.297(1.199 ~ 1.403), gender 1.670(1.371 ~ 2.034), glycated hemoglobin 1.414(1.026~1.948),insulin analogue 1.457(1.084 ~ 1.957), drinking 1.770(1.321 ~ 2.371), BMI 1.137(1.085 ~ 1.192), family history of diabetes mellitus 1.531(1.037 ~ 2.260) and family history of cancer 4.853(3.602~6.537). Conclusion:1.Hospitalization of patients with type 2 diabetes, the malignant tumor detection rate is increased significantly;2.In male patients with type 2 diabetes of top three cancer are colon cancer, lung cancer, liver cancer, beside in female the top three cancer are colorectal cancer, breast cancer,endometrial cancer;3.Older age, male, obesity, Higher glycated hemoglobin,insulin analogue hypoglycemic way, drinking, having a positive family history of diabetes mellitus and cancer, are risk factors for diabetic patients undergoing malignant tumor;4.Diabetic patients should pay attention to early screening of malignant tumor. |
