Objective: Viral encephalitis(VE)is the central nervous system inflammatory caused by viral infections,common in children.Many viruses can lead to infections of the central nervous system.The onset of different viruses,transmission,geographical distribution,prevention and treatment is different.At present the diagnosis of viral encephalitis rely mainly on clinical diagnosis,including clinical manifestations and the examination of EEG,cerebrospinal fluid and neurological imaging.In china restricted by etiological diagnosis level,relevant epidemiological and etiology data are lacking.The 184 children with viral encephalitis were retrospectively analyzed to further understand the epidemiological characteristics of viral encephalitis in children in Hebei Children’s Hospital in 2014,and provide scientific evidence for diagnosis,prevention and cure of viral encephalitis.Methods: We collected the general information,clinical data,serum and CSF samples of all the patients hospitalized in Hebei Children’s Hospital neurology department with the diagnosis of viral encephalitis from November 2013 to December 2014.Real-time fluorescence quantitative PCR(RT-PCR)was performed to detect the viral of cytomegalovirus(CMV)epstein-barr virus(EBV)human herpesvirus type6(HHV6)varicella-zoster virus(VZV)herpes simplex virus1(HSV1)herpes simplex virus2(HSV2)human enterovirus(HEV)mumps virus(MV)and Japanese encephalitis virus(JEV).Enzyme-linked immuno sorbent assay(ELISA)was applied to test the IgM and /or IgG antibody of rubella virus(RV)mumps virus(MV)tickborne encephalitis virus(TBE)and dengue virus(DEN).Statistically analyzed the virus detection positive rate and the distribution to Compare the epidemiological difference between enterovirus encephalitis and and the overall viral encephalitis.Results:184 casess of viral encephalitis(male 64.13%,female 35.87%,the male:female ratio was 1.78:1 and the ages of most were 3-7 years)were mainly come from 4 regions: shijiazhuang,xingtai,baoding and hengshui.The peak time of incidence was from April to September.In the ealier of disease 25.95% CRP was higher than normal,64.89% electroencephalograms of patients showed abnormal and cells in CSF was increased.The positive detected rate of virus in 120 patients was 50.83%(61/120);the positive rate of HEV,EBV,HSV1,MV,HHV6,CMV detected rate was separately 37.50%(45/120),8.33%(10/120),2.50%(3/120),1.67%(2/120),1.67%(2/120)and 0.83%(1/120).The positive cases of Virus antibody testing for MV-IgM,MV-IgG,RV-IgM was separately 4,16 and 3 cases.Statistics show that enterovirus was the major pathogen of Viral encephalitis.The onset time of enterovirus encephalitis was more concentrated.45 cases of viral encephalitis caused by enterovirus infections are mainly distributed in summer,from May to July,and the age and regional distribution are consistent with the overall viral encephalitis.Conclusion : HEV was the main pathogen of virus encephalitis in children,followed by EBV,HSV1,MV,RV,HHV6 and CMV in Hebei Children’s Hospital in 2014.Surveillance of virus encephalitis should be strengthened. |
