Objective: to respectively of endoscopic variceal ligation combined with somatostatin and pure endoscopic set ligation operation in the treatment of esophageal variceal bleeding were retrospectively analyzed to explore the two methods of esophageal varices bleeding effect and complications of treatment research to in order to promote the further understanding of EVB clinical treatment and evaluate the effect and application value of two methods, and the further complication prevention. Methods: This study retrospectively analyzed from 2013 1 to September2015 in Guangxi University of traditional Chinese medicine, the First Affiliated Hospital of digestive and liver disease from Department of internal medicine cirrhosis esophageal variceal bleeding in 106 patients and hospitalized patients were randomly divided into treatment group and control group, based on the basis of Department of internal medicine, conservative treatment, the treatment group was given endoscopic ligation combined with somatostatin treatment, control group was treated by endoscopic ligation, were recorded in patients with clinical data, the average number of times, ligation ligation ring number, postoperative bleeding, rebleeding, death, complications, using statistical methods for processing and analysis of data, combined with the clinical effect evaluation. Results: 106 cases were randomly divided into two groups.Endoscopy combined with drug group of 54 cases,44 were male,10 female patients, age in28 ~ 67 year old;endoscopic group a total of 52 cases,45 cases were male and 7female patients, age at the age of 32~66.Patients in the two groups in age, gender, etiology, classification of liver function, esophageal varices and red syndrome contrast p value > 0.05,the difference no significant, the two groups had comparability.Hemostasis success rate was 96.3%,92.3% respectively, the elimination rate of 3.7% and 7.7% respectively, average each ligation ring numbers were 6.74 + 0.620,6.83 + 0.474,delayed rebleeding rate were 9.3%,15.4% and 5.56%,9.61% mortality, respectively,P > 0.05,the difference was not statistically significant, two groups in the hemostatic effect, varicose veins elimination, each ligation ring number and delayed rebleeding, death, there is no significant difference. Two groups of significant efficiency were 29.6% and11.5%,average treatment times and course were 2.04 + 0.334,2.25 + 0.682,early rebleeding rate was 1.9% and 13.5% respectively,P < 0.05,the difference was statistically significant, endoscopy combined with drug group in the speed of onset than in the endoscopic group faster, you can reduced set of ligation in the treatment of times and reduce early rebleeding occurred.According to the data of the two groups that adverse reaction of two groups after operation, postoperative fever, dysphagia sense is more common adverse reactions, but two groups in postoperative fever, chest pain, dysphagia, nausea and vomiting was used to compare the incidence of,P > 0.05,the difference was not statistically significant. Conclusion: through this study, for liver cirrhosis patients with esophageal variceal bleeding were compared with endoscopic ligation combined with somatostatin group and simple endoscopic ligation group two group the rate of hemostasis, average ligation times and average ligation ring number, rebleedingrate, complications and mortality to the following conclusions: endoscopic variceal ligation combined with somatostatin in the treatment group, the average hemostatic effect of ligation ring number, varices elimination rate, complications, mortality and endoscopic group had no significant difference, but compared with endoscopic ligation therapy can reduce ligation treatment times, and a greater extent reduce the cases of varicose veins, and the effect is more obvious.Early rebleeding rate was significantly lower than that in the endoscopy group. In this regard, the clinical selection can be carried out on the two kinds of treatment methods according to the different situations of patients. |
