PurposeThis study to our hospital RICU with retrospective analysis on 74 patients of clinical data with ARDS, aimed at exploring the predisposing factors,assessment on severity,treatment and reaction,survival rate and mortality,and the prognosis related factors.MethodPatients who were admitted to the Second Affiliated Hospital and First of Chongqing Medical University from January 2013 to June 2014 and clearly diagnosed as ARDS were analyzed retrospectively. We collected the clinical data including general information, treatment and prognosis of disease.Resultsl.In the 74 patients with ARDS,54 patients of male,20 patients of female,mean age(72.21±10.24). According to the Berlin definition,10 are mild ARDS,30 are mediate ARDS,34 are severe ARDS.Among the patients,43 died(68.1%),31 alive(41.9%).2. Among the patients,pneumonia is the major cause of ARDS.3.Serum lactic acid level has positive correlation with disease severity,but there is no statistical differences between survival and non-survival group.4.Procalcitonin,APACHEII scores have positive correlation with disease severity,meanwhile,there is statistical differences between survival and non-survival group.5.Oxygenation index has negative correlation with disease severity,there is statistical differences between survival and non-survival group in mild or mediated ARDS,but there is no statistical differences between survival and non-survival group in severe ARDS.6.Among the 74 patients,sputum culture of 20 patients are positive,blood culture of 6 patients are positive.intravascular catheter culture of 1 patient is positive.Baumani is the most common pathogen.7.17 of 74 patients gastric content occult blood test are positive,patients are often accompanied with other organ failure:circulatory failure(6)}renal failure(13),liver failure(3),coagulation function failure (2),central system failure(1).8.62 of 74 patients use invasive mechanical ventilation,20 patients use non-invasive mechanical ventilation,10 patients use invasive and non-invasive sequential therapy. All the patients use antibiotics,51 patients use glucocortioid in different extend.2 patients use dialysis,l patient is give chemotherapy because of multiple myeloma.Conclusions1. Among the patients,the mortality of mild,mediated,severe ARDS is 3%,53.3%,70.5% respectively,higher than the foreign study.2.APACHEII score,serum lactic acid level,Procalcitonin,and oxygenation index are correlated with disease severity,they can be used to assess the prognosis of the disease.3.Baumani is the most common pathogen in sputum and blood culture.4. ARDS is always accompanied with other organ failure,and kidney is the most commonly involved.
