Diabetes and dyslipidemia are risk factors for cardiovascular disease. The prevalence rate of diabetes in adults in China is 11.6%, however, only 30.1% of people know they have it. But only 39% of patients with dyslipidemia are treated by lipid-lowering therapy. On the other hand, in China, deficient and unevenly distributed medical resources can not be solved for the lack of cooperation mechanism, macro policy and health spending which have scuppered process of health-care reform. Therefore, online sub-health service platform based on various sense technology for preventing and monitoring chronic diseases would be a good way to solve these problems.This thesis gives a complete analysis based on the existing problems, learning from foreign advanced technology and experience of online health service platform, in order to establish online health service platform suitable for our country. This platform would be beneficial to the people who want to get health-care, the institutions which want to provide health care resources and service. What’s more, on this online sub-health service platform the sub-health prevention and modern information science and technology could be combined together for preventing and monitoring chronic diseases inorder to reach the concept that health security can be effective before diseases invading.For the human being to have the privilege to say “no” in front of diseases, and to let the multi-parties on the platform to have a “win-win” situation, the human health needs to be safeguarded beginning from its sources. Meanwhile, to conduct impact statements with key technology and structure of this online sub-health service system platform, and make key implementations and recommendations of the online sub-health services platform.
