Background:Road traffic injury is one of the most important causes of death and disability.It is a public health problem that shouldn't be ignored.After taking various preventive and control measures,the incidence of road traffic accidents has decreased,but the amount of electric-bike road traffic accidents is increasing.Electric bike has a rapid development in recent years in Chinese market,the amount of Chinese electric bike is also increasing rapidly.In 2010,the amount of China's electric bikes had reached to 120 million,in 2013,the amount breakthrough to 200 million,in 2017,the amount reaches to 250 million.The increase in the number of electric bike and the number of accidents associated with the phenomenon of convergence,the number of casualties caused by electric bike-related accidents is also increasing,but it is ignored by the people of China and abroad.The studies of electric bike related road traffic accidents are mostly simple reporting data,or only a simple descriptive study.Systematic researchest on the electric bike road traffic injury is lack.The epidemiological data of the factors affecting the traffic injury of electric bike are very scarce.To solve this problem,we conducted a road traffic injury survey of electric-bike riders in Yi Xing-a Chinese city with high electric-bike self-holding capacity.The purpose of this study is to analyze the current situation and risk factors of electric bike-related traffic injuries through cross-sectional study and case-control study,and put forward the relevant epidemic prevention measures.In order to enrich the epidemiological data of electric bike-related road traffic injury,to provide a scientific basis of the advice about prevent and reduce the occurrence of electric bike road traffic injury.Objectives:1.To understand the basic demographic characteristics,behavior of electric biker,physical and psychological status of electric biker,the type and function of electric bike,to understand the current situation of road traffic damage of electric bike,and to provide the basis for studying the influencing factors of electric bike-related road traffic injury.2.To explore the influencing factors of electric bike-related road traffic injury.3.To put forward prevention and control strategies of electric bike-related road traffic injury.Methods:1.Cross-sectional study:Using convenient sampling method,select 45 communities in Yixing,select part of the 16-year-old electric biker from these communities as the respondents,develop a questionnaire survey,the survey mainly includes:basic information of electric biker,the basic situation of electric bike,the situation of riding when E-biker riding electric bike,the situation of electric bike-related accident,the situation after the accident.2.Case-control study:Using 1:3 matched case-control study method,all data undergo single factor logistic regression analysis and multiple conditional logistic regression analysis,choose the respondents who had occurred accident as the case group,and respondents who hadn't occurred accident in the past year as the control group,respondents' gender,age(±5)and occupation in control group is the same as respondent in case group.3.Prevention and control strategies:Refer to "5E" injury prevention strategy,this study proposed electric bike road traffic injury prevention and control strategies from four aspects:electric bike itself,electric biker,roads and riding environment.On the basis of the above research,combined with the electric bikers' interviews,and refer to relevant literature at home and abroad.Results:1.Cross-sectional study:A total of 3200 community residents in Yixing city were surveyed,3151 valid questionnaires were collected,the effective rate was 98.47%,and the accident rate was 15.99%(504/3151).Electric bikes in Yixing have severely exceeded the limit,electric bikers' knowing rates of related road traffic regulations is low,running a red light,driving in the motor vehicle lane,reverse driving and other unsafe driving behaviors are universal.The electric bike-related road traffic injuries of Yixing mainly occurred in October(16.87%).The main types of injury were abruptness(87.04%)and fracture(8.95%).The main injury parts were head(10.19%)and limb pelvis(27.16%).The main cause of the accident is based on the driver(43.65%).2.Case-control studySingle factor logistic regression analysis found that two electric biker's basic situation include history of accident,accident risk,etc.;eight electric biker's health status include sleep condition,sleep time,arthritis,rheumatism,heart disease,etc;two drinking conditions;seven mental states include fatigue,anxiety,helplessness,etc.thirteen riding behaviors include running red lights,drunk riding,answer the phone or call others,etc.laws and regulations include riding lane is non-motor vehicle lanes,under 16 years of age can not ride electric bike,etc.the type of vehicle;whether with pedal riding function and riding skills,a total of 38 variables are significant risk factors for the single factor analysis(P<0.05).Multiple conditional logistic regression analysis found that history of accident(OR = 3.10,95%CI:1.92?5.01),accident risk(OR = 5.84,95%CI:4.11?8.28),"sometimes" and "often" feel anxiety(OR = 1.84,95%CI:1.37?2.46;OR=2.16,95%CI:1.41?3.32),poor riding skills(OR = 5.87,95%CI:1.94?17.75),"sometimes"wearing a headset to listen to music(OR ? 1.74,95%CI:1.12?2.71),"Often" carrying adult passengers(OR ?1.69,95%CI:1.20 to 2.40),"sometimes" and "often" riding in motorway(OR = 1.84,95%CI:1.30?2.59;OR=1.96,95%CI:1.36?2.84),"ofen" riding on the opposite(OR = 1.57,95%CI:1.04?2.38)are the main risk factors of electric bike-related road traffic accidents.3.Prevention and control strategies:combined with the characteristics of electric bike road traffic injuries,refer to the "5E" injury prevention strategies,mainly from these five aspects to put forward road traffic injury prevention and control strategies:the control of excessive electric bike,the strengthen of management of unsafe riding behavior,the improvement of electric biker's safety knowledge and safety awareness,the improvementof road and riding environment and the development of electric bike road traffic injury monitoring.Conclusion:1.Accident rate of electric bike road traffic is high in Yixing,it is one of the important public health problems which has been a threat to the lives of residents.2.History of accident,accident risk,anxiety,poor riding skills,wearing a headset to listen to music,carrying adult passengers,riding in motorway,riding on the opposite are the main risk factors of electric bike-related road traffic accidents.3.The control of excessive electric bike,the strengthen of management of unsafe riding behavior,the improvement of electric biker's safety knowledge and safety awareness,the improvementof road and riding environment and the development of electric bike road traffic injury monitoring are effective measures to reduce electric bike road traffic injury.
