Objective: By comparing the operation time, cosmetic effect, postoperative satisfaction,temporary facial paralysis skin numbness around ear area prate of Frey syndrome, salivary Fistula of v-shaped incision between v-shaped incision and conventional S-shaped incision in surgeries of benign lesions around the parotid gland region,we explored the clinical application value of v-shaped incision in removal of benign lesions in parotid region.Methods: By collecting patients who had received surgeries of benign lesions in parotid region during January 2014 to January 2015 at Zunyi Medical College Affiliated Hospital of Stomatology Department of oral and maxillofacial surgery, 52 patients were divided into two groups, group A:19 cases of v-shaped incision, group B:33 cases of conventional s-shaped incision.Operation method:In addition to the different surgical incision, the surgical method included anterograde anatomy of the facial nerve trunk, the protecting of the great auricular nerve, and the removal of tumor and tissue 0.5-1.0cm surrounding the parotid region.According to the defect situation, tissue flap of the parotid gland, sternocleidomastoid flap and dental restorations membrane would be used to cover the surface of parotid gland to prevent the Frey syndrome. After surgeries,the operation time, facial paralysis, skin numbness, tumor recurrence rate, and salivary fistula of all patients were carefully recorded, then we ran the minor,s test on patients, compared surgical scars, and made surgical satisfaction questionnaire in postoperative 6 months.Results:1.The average operation time of V-shaped incision was 124.2 min, and that of conventional s-shaped incision 120.7min, there existed no statistical difference(P > 0.05); 2.The average length of v-shaped incision was 6.58 cm, and that of conventional s-shaped incision was 10.28 cm, statistical difference existed(P < 0.05);3.Comparing by the visual effect before and after surgeries, v-shaped incision was hidden, with good cosmetic effect;4.Postoperative satisfaction:the average satisfaction degree of v-shaped incision was 21.95, and that of conventional s-shaped incision was 20.39,there showed statistical difference(P < 0.05);5.The facial paralysis:temporary facial paralysis rate of v-shaped incision rate was 10.5%,and the rate of conventional s-shaped incision was 18.1%, no statistical difference showed(P > 0.05); 6.skin numbness around the ear area:the skin numbness rate of v-shaped incision was about 21.1%, and that of conventional s-shaped incision was15.2%,no statistical difference showed(P > 0.05);7.Frey syndrome: the incidence of v-shaped incision was 5.2%, and that of conventional s-shaped was 6.0%, there existed no statistical difference(P > 0.05); 8. Only 2 cases of salivary leak were observed in v-shaped incision group.Conclusion: Comparing to conventional s-shaped incision, under the premise of meeting the need of surgical operation and no increasing of surgical complications,v-shaped incisionshows characteristics location, less postoperative scar, good cosmetic effect.
