Warming-promotion Combined With Contralateral Needling Method Clinical Research On Treatment Of Scapulohumeral Periarthritis Of Wind-Cold-Damp Type

Objective: The purpose of this thesis is to observe the clinical effect of warming-promotion method of acupuncture combined with contralateral needling method therapy on shoulder periarthritis. After comparing with twirling needle method, the advantages of warming-promotion method of acupuncture combined with contralateral needling method therapy in stopping pain, relieving complications, shortening course of treatment, selecting precise acupoint, and shortening the time of retaining needles were verified. It provides a simple but effective clinical plan in treating shoulder periarthritis.Methods: Selected cases in gansu university hospital of traditional Chinese medicine in acupuncture outpatient and the inpatient patients with periarthritis of shoulder with the method of random number table.72 patients of shoulder periarthritis were divided into two groups, the one is the treating group(warming-promotion method of acupuncture combined with contralateral needling method therapy group of 36 patients) while the other one is the control group(twirling needle method group of 36 groups). All the patients were treated according to the experimental plan. In the treating group, the points of Jianyu,Jianzhen,Jianliao,Fengchi,Hegu and Tiaokou(contralateral point), were chosen as the treating points. In the comparing group, the patients were treated on the same points,but Tiaokou is the point of affected side. The time of keeping needles in the two groups was 30 minutes. The two groups were treated once every day, and ten times is a treating period. The patients must be treated with three periods. VAS grading and statistics and shoulder-joint-function grading and statistics were accomplished respectively before treating, after the first treatment and after three periods. And after the whole period of treatment was finished, the patients were accessed in the curative effect. Finally, the clinical data index collected was analyzed with statistics.Results: 1. After a period of treatment was completed, a comparison about VAS soreness grade on patients’ shoulders was accomplished respectively before and after a period of treatment and three periods treatment was finished. The result was P<0.05, there was difference with statistical significance. In the comparison of VAS grading of instant pain-relieving after the first treatment, the excellent rate of treating group was 83.3%, and the excellent rate of control group was 61.1%. The comparing result was P<0.05,there was difference with statistical significance. And the Ridit analysis on the two groups’ rating data showed the result with P<0.05, there was difference with statistical significance. 2. After a period of treatment and three periods treatment was completed, the inter-group integration comparison show the result was P<0.05, there was difference with statistical significance. Compare the patients grade of their shoulder-joint function before and after treatment, the result was P<0.05, there was difference with statistical significance. 3. After a period of treatment and three periods treatment was completed, the inter-group integration comparison show the result was P<0.05, there was difference with statistical significance. 4.In comparing the total curative effect, the treating group was 97.2%, and the control group was 77.8%. The result was P<0.05,there was difference with statistical significance. Ridit analysis on ranking data showed the result was P<0.05, there was difference with statistical significance.Conclusion: 1.Both warming-promotion method of acupuncture combined with contralateral needling method therapy and common ones have good clinic curative effect on shoulder periarthritis. But warming-promotion method of acupuncture combined with contralateral needling method therapy is better than reinforcing and reducing by routine acupuncture therapy. 2.In the respects of improving symptoms, signs and restricted upper-limbs movement, warming-promotion method of acupuncture combined with contralateral needling method therapy is better than reinforcing and reducing by twirling needle method.
