Background Uterine fibroids and adenomyosis are common uterine diseases,currently radical cure way is hysterectomy,but do not fit the patients who want to keep the endometriosis or fertility requirements; AWE it refers to any endometriosis lesions which occurrence in the abdomen,Currently the local excision is the preferred solution for the clinical treatment,but it is an invasive treatment.HIFU as a non-invasive treatment,realized minimize harm or non-invasive treatment for gynecological diseases,widely used in the field of gynecology,its treatment of uterine fibroids and adenomyosis is safe and effective,Expected to become the first-line treatmen of uterine fibroids and adenomyosis;In recent years, about HIFU treatment AWE showed that it has better efficacy,fewer side effects, the treatment process is relatively simple 〠fast, good patient acceptance and so on.Theoretically that ultrasound enter into the human body may bring about reflection, refraction, scattering By body tissue interface,coupled with the sensitivity differences impact of tissues and organs,results in that when HIFU treatment the target generation may addition to damage outside,in particular tissues or organs on the sound channel.Objective1.Research the change of abdominal wall before and after HIFU treatment,analysis the related factors of HIFU treatment the uterine fibroids,adenomyosis, endometriosis.2.Discuss of the clinical efficacy of HIFU treatment of abdominal wall endometriosis.Methods1.Retrospective analysis one hundred and sixty-three patients with uterine fibroidsã€adenomyosis or AWE who underwent HIFU ablation in the First Hospital of Chongqing Medical University.Research the change of abdominal wall before and after HIFU treatment,analysis the related factors of HIFU treatment the uterine fibroids, adenomyosis, endometriosis.2.Retrospective analysis thirty-three patients with AWE who underwent HIFU ablation in the First Hospital of Chongqing Medical University.The patients were followed up at 3 month, 6month and 12 month after HIFU ablation, and the symptoms were recorded.Result1.Abdominal reactive changes occurred in forty-four patients after treatment,The reactive changes of the abdominal wall after HIFU treatment is unrelated with the disease is uterine fibroids or adenomyosis(p>0.05).Lesions to the skin 〠therapeutic doses have no statistically significant difference( p>0.05).After HIFU treatment twelve AWE patients the abdominal reactive changes,it have no statistically significant difference with the irradiation timeã€therapeutic dosesã€maximum power(p>0.05). The shorter lesions to the skin easier lead to the abdominal wall response to change(p>0.05).2.After 3 monthsã€6 months and 12 months follow-up, thirty-three patients with abdominal cyclical pain symptoms gradually improved, lesion volume gradually absorbed reduced, and contrast before surgery difference was statistically significant(p>0.05).effective rates were 78.8%, 84.8%,87.9%.Conclusion1.Most patients had no significant effect on the skin after HIFU treatment of uterine fibroids, adenomyosis, few cases occurred abdominal reactive changes, all can be restored in the short term, didn’t degrade the quality of life. The occurred of abdominal reactive changes after HIFU treatment of AWE have negative routine wiht the skin lesions. That means closer target of the treatment area and skin easier lead to abdominal reactive changes.2.It was worthy of recognition that short-term efficacy of ultrasound ablation treatment of abdominal wall endometriosis, but as the group report just have few cases, the lack of long-term efficacy evaluation, it need a large sample, long-term evaluation to assess the value of HIFU in the treatment of abdominal wall endometriosis. |
