The Clinical Relationship Between Intestinal Metaplasia Gastric Mucosa And Helicobacter Pylori Infection

Objectives:To analyze the curative effect about HP infection in patients with chronic gastritis and inestinal metaplasia by comparing conventional quadruple therapy in comlination with Bora-yeast with conventional quadruple therapy in combination with hericium extract granules,the symptoms of patients,and the incidence of adverse reactions.Subjects and Methods:1.The subjects are collected from October 2014 to June 2015 in the Nutrient-Laden medical clinic of the First Affiliated Hospital of Yanan University, who are infected as chronic gastritis and inestinal metaplasic by 14C-UBT breath test,the endoscopic and pathological diagnosis, and the total number is 130. All the elected patients are qualified for the criterion.They are divided into 2 groups according to the Hp infection situation after passing eliminating the standard of checking.The two groups are the hp feminine gender group(group A, 26 people)and Hp muscular gender group( group B, 104people).The group B is divided into three groups again randomly,which include B1 group of 35 people,B2 group of 35 people and B3 group of 34 people.2.Four groups of patients with specific plan and course are as follows:A group(Hp Negative groups): Hericium erinacecus extract granule, treatment 90d; B1 group(Conventional four transmitte): Omeprazole enteric capsules + Amoxicillin capsule +Clarithromycin tablets + Particles of colloidal bismuth pectin, treatment 10d;B2course(Conventional four transmitte + Hericium erinacecus extract course): Omeprazole enteric capsules + Amoxicillin capsule + Clarithromycin tablets + Particles of colloidal bismuth pectin course 10 d. Hericium erinacecus extract granule, treatment 90 d.B3 course(Conventional four transmitte + the Laplace yeast group): Omeprazole enteric capsules +Amoxicillin capsule + Clarithromycin tablets + Particles of colloidal bismuth pectin,treatment 10 d. The medicine methord: Omeprazole enteric capsules 20 mg bid in the morning and once: Amoxicillin capsule 1000 mg bid after a meal to take.Three months later, collecting again the patient’s clinical symptoms, rating according to the grading standard, and analysising the symptoms of patients.After six months, the review of 14 c-UBT several endoscopic biopsy pathology examination, to know the situation of HP eradication in patients and analysis of the incidence of adverse reactions of them.Results:1.The general condition of these four groups of the patients, including gender, age,smoking, drinking, acrid food intake.Though statistics analysis and inspection, there were no statistically significant difference.(p>0.05)2. Four groups of clinical curative effect, the total effective rate, generally speaking,there are differences between groups.(X2=10.218 p<0.05). Compared between two groups, the group of A and B2, the group of A and B3, p<0.0083, the difference has statistically meaning.While others, which meet the need of p>0.0083, are meaningless(a=0.0083).There’s statistical significance of differences. Contrasted with other groups(P>0.0083), there’s no statistical significance.3. There’s no statistical significance by contrasting B1 and B3 in Hp eradication rate.(X2=0.212 P>0.05).4. There’s statistical significance by contrasting B1 and B2 in therapeutic effect of intestinalisation.(X2=7.014 P>0.05).5. All four groups were statistical significant in Adverse Reaction.(X2=11.217P<0.05), and the following specific compared data between 2 groups was no statistical significance: Group A and B2(x2=0.683 P>a′); Group A and B3(x2=0.148 P>a′); Group B2 and B3(x2=0.251 P> a′). There’s statistical significance in the following comparation between 2 groups: Group A and B1(x2=7.329 P< a′); Group B1 and B2(x2=8.035 P< a′;Group B1 and B3(x2=8.549 P<a′ a′=0.0083).Conclusions:1. Four groups of drugs all can relief the clinical symptoms, but the clinical effects are more obvious with the eluate of Saccharomyces Boulardii or Hericium erinaceus.2. There is no obvious difference between regular quad and Saccharomyces Boulardii in the efficient of eradicating Hp, but saccharomyces boulardii can reduce the incidence of adverse reaction.3. Hedgehog fungus extract granules uniting regular quad to eradicate Hp can reduce the occurrence of severe adverse reaction. And there is reverse effect for gastric mucosa intestinal Metaplasia.
