The Effectiveness Of Combined Or Single Use Of Ulinastatin And Low Molecular Weight Heparin For Treating Acute Pancreatitis In China:The Meta Analysis

Objective: To valuate systematically the clinical effect of acute Pancreatitis with the different drugs such as Ulinastatin, Low molecular weight he Parin(LMWH)and the combination of them by the Meta-Analysis.Methods: Com Puter retrieval CBM, VIP, CNKI, WANFANG database of ulinastatin, low molecular weight he Parin alone or used for the treatment of acute Pancreatitis clinical randomized controlled trial(RCT), and the recourse of the literature of loss of original data. Extract information and data, the a PPlication of Meta analysis software Rev Man5.3 were analyzedResults: The grou P with the combination of Ulinastatin and Low molecular weight heparin(LMWH) include 6 RCT: Com Paring with the common treatment, A total of 4 outcome which are cure time, white blood cell count, CRP, and Severe Forming have statistical significance(P<0.05),and 2 outcome which are o Peration rate and mortality have no statistical significance(P >0.05). Comparing with the group with the Low molecular weight heparin(LMWH), the CRP and the cure time have statistical significance(P<0.05),but Severe Forming have no statistical significance(P >0.05). Comparing with the group with Ulinastatin, cure time, APACHE- II score, CRP, and CTSPN have statistical significance(P < 0.05),but Severe Forming have no statistical significance(P >0.05). Com Paring with the group with octreotide, APACHE- II score, o Peration rate, Complication,hos Pital stay and CTSPN have statistical significance(P < 0.05).The group with low molecular weight heparin(LMWH) include 17 RCT: Comparing with the common treatment, A total of 8 index which are cure time, CRP, Severe Forming, Content of serum amylase, Complication, operation rate,mortality and partial pressure of arterial oxygen have statistical significance(P < 0.05),but APACHE- II score have no statistical significance(P >0.05). The group with Ulinastatin include 24 RCT: A total of 12 index which are cure rate, mortality rate, operation rate, total effective rate, abdominal pain disappeared time, hospital stay, blood amylase return to normal time, CRP, APACHE- II score, TNF- a, IL-6, adverse reactions have statistical significance(P < 0.05).Conclusion:(1), LMWH and UTI treatment for AP has significant clinical effect, but in the comparison of the total effective rate, UTI is better than that of LMWH.(2), combined with LMWH and UTI both have synergy in function link and the mechanism of factors, both combination than conventional treatment alone/AP clinical effect is more significant, can be used as medicine for the treatment of AP;(3), as a result of LMWH and UTI combined treatment literature quantity is less, the quality is low, the need for more large sample randomized blinded trials, more stringent multicenter clinical trial observation, provide more secure and reliable for clinical use of evidence.
