Objective:To summarize clinical efficacy of Prof. Yao Zupei treating primary hypertension by combining traditional Chinese medicine with western medicine and sum up his clinical experience, aims to provide guidance and reference for clinical treatment.Methods:A total of 59 outpatients and inpatients with primary hypertension of Nantong TCM hospital were collected by learning from Prof. Yao Zupei in clinic. Detailed diagnosis processes of Prof. Yao Zupei were persistent recorded for 4 weeks. Total effective rate and curative effect of TCM syndrome in all of patients after treatment were assessed by TCM syndrome scores table and meanwhile to evaluate total rate of clinical antihypertensive efficacy in partial patients still with high blood pressure. Analysis of syndrome differentiation and medication rule on hypertension from Prof. Yao Zupei, copied clinical prescriptions and attainment experience from his lessons were combined to sum up his clinical experience of TCM syndrome differentiation and treatment.Results:TCM syndrome scores of 59 patients with hypertension after treatment were significantly reduced than before (P<0.01). The clinical total effective rate was 83.05%. The total rate of antihypertensive efficacy of 45 patients with poor control of blood pressure after treatment was 80.00%. There was no adverse event in the process of treatment. Statistical results of syndrome differentiation in 59 cases of showed that the most common syndrome was hyperactivity of liver qi, accounting for 64.41%(including liver depression and qi stagnation accounting for 10.17% and extreme hyperactivity of liver qi accounting for 54.24%, while there were 42 cases of blood stasis from the all types of syndromes, accounting for up to 71.17%. Chinese medicinal herbs for calming the liver to stop endogenous topped the list of statistical classification of herbs, followed by heat clearing herbs and invigorating the blood and removing blood stasis herbs. Chinese medicinal herbs commonly used by Prof. Yao Zupei was achyranthes bidentata, radix paeoniae alba, prunella vulgaris, uncaria rhynchophylla, radix paeoniae rubra, rhizoma gastrodiae, radix puerariae, concha haliotidis and curcuma aromatica, the main action of which was to clear, tranquilize the liver and activate the blood, what is more, these herbs was mainly cold in nature, bitter and pungent in flavor and attributive to the liver meridian.Conclusion:The theory of "double heart" was highly valued by Prof. Yao Zupei, who paid attention to the relationship between psychological changes and blood pressure in clinical treatment of hypertension. He held that hypertension was closely related with the liver lesions and disharmony of qi-blood was an important pathogenesis, therefore, demonstration of hypertension showed more sthenia syndrome and less asthenia syndrome in clinic, with syndrome of hyperactivity of liver qi and obstruction of blood circulation. In terms of treatment, Prof. Yao Zupei advocated that it had to start with regulating liver qi and invigorating the blood and removing blood stasis to harmonize qi and blood, which was contribute to lower blood pressure and especially for improvement of clinical symptoms and playing role in synthetically regulation effect from multi-level and multiple sections and multiple targets in the treatment of hypertension by the traditional Chinese medicine. |
