Objective:To investigate the clinical efficacy of Xianling Gubao capsules combined with extracorporeal shock wave for the treatment of lower extremity long bone nonunion, provide clinical evidence for the treatment of lower extremity long bone nonunion.Methods:From February 2014 to February 2015,60 patients who were diagnosed with lower extremity long bone nonunion were randomly divied into ESW group(extracorporeal shock wave) and Combined group(Xianling Gubao capsules combined with extracorporeal shock wave) according to their admission sequence by SPSS 18.0 software.30 patients in each group, including 34 males,26 females, mean age was 38.53+6.12 (24-50 years old).In the present study, we choose prospective study. Extracorporeal shock wave therapy device was used by ESW group, we selected three points as the reference point around the fracture line and those points must avoiding internal fixation treatment side, avoiding large blood vessels and nerve-intensive areas,.firstly, we put the therapy device over the treatment site, then we adjust the impact energy of tibia to3.5 bar and adjust the impact energy of femur to 4.0 bar, frequency:6Hz, Once weekly treatment, continuous treatment for 4 weeks.The combined group was treated with extracorporeal shock wave and Xianling Gubao capsules 1.5g twice every day. Respectively recording VAS score, weight-bearing limb situation ratings, X-ray bone callus Lane-Sandhu score, nonunion healing time and the number of nonunion after treatment of 3month,6 months and 12 months, and then using SPSS 18.0 software for statistical analysis.Results:Mean follow-up time was 12.5 months (12-14 months). During the period of follow-up, the ESW group and Combined group had no vascular, nerve damage and other adverse events occured.3 patients who using shock wave therapy feel slight pain, relieve pain a week after adaptation,5 patients with oral capsules Xianlinggubao cause constipation, give them soak 10g senna 2 times and ease. There was no significant difference of VAS pains and Weight-bearing limb situation between ESW group and Combined group after the treatment of 3 month(P>0.05). The VAS pains of Combined group was better than ESWS group after the treatment of 6 months and 12 months(P<0.05). The clinical fracture healing time, Weight-bearing limb situation,X-ray bone callus Lane-Sandhu score of the Combined group were significantly better than the ESW group(P<0.05).Conclusion:Xianling Gubao capsules combined with extracorporeal shock wave for treatment of lower extremity long bone nonunion patients can ease limb pain, promote clinical healing of nonunion. The effect of Combined Group is better than the use of ESW in the treatment of lower extremity long bone nonunion. Therefore, Xianling Gubao capsule combined ESW is an effective treatment for lower extremity long bone nonunion method. |
