The Observation Of Clinical Effects Of The Tonifying Kidney, Invigorating Spleen And Cooling Blood Decoction In The Treatment Of Chronic Immune Thrombocytopenia

Objective:To observe the clinical effects of tonifying kidney, invigorating spleen and cooling blood decoction in the treatment of chronic immune thrombocytopenia (CITP).Methods:The 20 patients with hospital or outpatient between February 2014 and March 2016 in Jiangsu Province of TCM were involved and divided into 2 groups.10 patients were in the group of pure Chinese medicine (the former group), including Huangqi, Shegndi, Xianmao, Zhongjiefeng, Xianhecao, Qiancao, Dijincao, Huzhang, Baizhu, Fuling, Huangbo. Another 10 patients were in the group of combination of Chinese medicine and glucocorticoids or with other immunosuppressive agents (the latter group). Both of the 2 groups needed to be observed for at least 3 courses (each of 4 weeks). Clinical indexs included platelet counts, the dosage of glucocorticoid withdrawal, bleeding scores and bleeding grades. The effects were evaluated by the efficiency standards established in the fifth academic meeting of thrombosis and hemostasis in 1995. All data were analyzed by SPSS21.0 software.Results:There were no statistical differences in gender, age and course of the disease between the two groups(P>0.05). The overall response rates after each courses were 80%,80%, 90% respectively in both groups. Taken at the four observing time spots sperately, platelet counts in the former group were (34.30±17.42)×109/L、(47.20±24.61)×109/L、(41.10 ±20.94)×109/L、(58.90±33.01)×109/L while they were (30.50±24.30)×109/L、(44.70 ±25.96)×109/L、(52.70±31.43)×109/L、(76.00±61.09)×109/L in the latter group. Stastical difference wasn’t shown until the third course in the former group, while it appeared as soon as 1 course later in the latter group. The mean doses of prednisone declined from 18.38±13.92mg to 3.63±4.27mg(P<0.05), and 4 patients were free of glucocorticoids 3 courses later. There were 4 patients in each group with bleeding symptoms at the initial of treatment, while 3 courses later, only 2 patients in the latter group had bleeding symptoms. Both of them were in grade 2. No statistical difference was shown in the number of patients with bleeding symptoms between the two groups(P>0.05). Bleeding scores declined from 2.75±0.5 to 0.25±0.5 in the former group while they declined from 2.00±0.82 to 1.25±1.5 in the latter group.Conclusion:The study shows that the tonifying kidney, invigorating spleen and cooling blood decoction can effectively improve platelet counts in CITP patients, assist glucocorticoids withdrawal and release bleeding symptoms. It’s curative effect is exact.
