Objective:To observe the the changes of the airway resistance before and after the treatment of bronchodilator on the patients with mild and moderate acute exacerbation of chronic obstructive pulmonary (AECOPD),and what are the effects on pulmonary function and clinical symptoms.Methods:Choose the patients with mild and moderate AECOPD who were in our hospital during the 2014.12-2015.06, a total of 80 cases. Among them,there were 62 cases of male,18 cases of female,with an average age of 67.5 years old. All these selected patients conformed to the diagnosis of COPD in 2014 GOLD guide,and conformed to the definition of acute exacerbation in guide. According to the guidelines, give every one bronchodilator treatment (tiotropium bromide,18μg, inhale), treatment for ten days. Then before and after the treatment,successively did impulse oscillometry lung function (IOS), recording the values of total airway resistance R5, central airway resistance R20. the peripheral airway resistance R5-R20ã€Peripheral elastic resistance X5 and Resonance frequency Fres. And do pulmonary function testing(PFT), recording the values of forced expired volume in one second(FEV1), forced expired volume in one second percentage of expected value (FEV1%pred) and inspiratory capacity(IC). At the sametime,lead them to fill out the CAT questionnaire and score,to evaluate the improvement of patients’clinical symptoms before and after treatment.At the last,use SPSS17.0 to put all the values to calculate and conduct statistical analysis.Results:1 ã€After the treatment, the values of R5ã€R5-R20ã€X5 and Fres were Lower than before, and had statistic significance (P<0.05);However the values of R20 were amost the same as before,and no statistic significance(P=0.82).2ã€After the treatment, the values of FEV1〠FEV1%pred were few higher than before,but did not have statistic significance(P>0.05). But the value of IC were obvious increase than before, and had statistic significance (P<0.05).3ã€After the treatment,the CAT questionnaire score were declined than before,and had statistic significance(P<0.05).Conclusion:1ã€The mild and moderate AECOPD patients’peripheral airway resistance and total airway resistance were reduced after the comprehensive treatment of bronchodilator for major meanwhile the peripheral elastic resistance was decreased.2 The decrease of airway resistance made the value of IC increased, the lung ventilation function improved.3. Ultimately the decrease of airway resistance and the improvement of lung ventilation function,can relieve clinical symptoms,such as cough, cough phlegm and asthma after activity, making the living quality up. |
