Objectives:We aimed to examine the development of chronic atrophic gastritis (CAG) pathologically and explore its potential influencing factors.Methods:This retrospective study included 1312 patients who underwent gastroscopy during the period 2011-2015 at Sir Run Run Shaw Hospital, Hangzhou, China. They had undergone gastroscopy two times or more totally since the first time at Sir Run Run Shaw Hospital. Pathological and clinical findings were analyzed. Data collected included gender, age, length of follow-up period, family history of gastric cancer, history of Hp. infection, drug history, lifestyle history, including smoking and alcohol consumption, and the levels of serum PG I and PG II. Relative factors were statistically analyzed.Results:This study included 1312 patients of CAG,625 were male,24-79 years old, average of 58.0. The follow-up time ranges from 1.4-208.7 months.13 (0.99%) patients finally turned into cancers. Abdominal pain (51.83%) and abdominal distension (43.06%) were the most common clinical symptoms of CAG. 893 (68.06%) patients have history of Hp. infection.137 (10.44%) patients have family history of gastric cancer.291 patients have the history of long-term medicine intake. The patients of smoking or drinking are 163 and 173 respectively.32 patients have undergone serum PG test. Finally, Patients with the scores of positive development of atrophy, intestinal metaplasia and intraepithelial neoplasia are 389,408,22, respectively. The scores of negative development of Atrophy, Intestinal metaplasia and intraepithelial neoplasia are 513,657,75 respectively. The analysis results show that gastric mucosa was found to be pathologically aggravated by the history of Hp. Infection (OR=0.696, 95% CI 0.523-0.926, P=0.013) and old age (OR=0.998,95% CI 0.979-1.00, P=0.047; OR=0.982,95% CI 0.972-0.993, P=0.001). In the patients team, the PG â… , PG â…¡ and PGâ… /â…¡ are 50.1 (43.75,65.8) (ng/L),PG â…¡=7.15 (5.68,10.50) (ng/L),6.49 (5.48,7.91) respectively. And the PG â… , PG â…¡ and PG â… /â…¡ of control group are 94.8 (91.65,97.7) (ng/L),13 (11.35,13.6) (ng/L),7.45 (6.99,8.41) (p<0.;05)Conclusions:CAG is a slowly progressive disease. A history of Hp. infection is an aggressively factor of CAG. However It is also related to the physiological aging partly. The changes of PG â… , PG â…¡ level and the decreasing of PG â… /PG â…¡ ratio play an important role in the early diagnosis of atrophic gastritis as a test (P<0.05).
