| ObjectiveThrough literature research, summarize ancient physicians’clinical experiences about the etiology, pathogenesis and therapeutic principle of "Xiaoke disease", which is called diabetes mellitus(DM) today. Based on the real world research method, and collect the medical records of using syndrome differentiation to treat type 2 diabetes mellitus(T2DM) in Professor Li Saimei’s clinic, this thesis aims to discuss the curative effect of Traditional Chinese Medicine(TCM) in treating T2DM from two aspects, subjective symptoms and laboratory test index. Besides, analyze the clinical experience of Professor Li Saimei in the treating T2DM.MethodsThis thesis through literature review to summarize ancient physicians’ clinical experiences of "Xiaoke disease" from etiological factors and pathogenesis, therapeutic principles and methods. Based on real world approach, Observing the symptom score, HbAlc, FBG,2-hPG and other evaluating indicators before and after the treament to assess the curative effect of treatment. Explore professor’s clinical experience of treating T2DM by counting up the prescriptions.ResultsAncient literature research shows that Chinese medicine for "Xiaoke" has formed a relatively complete knowledge system. There also exist profound and original views, including etiology, pathogenesis, progression, prognosis, susceptible population, complications, therapeutic principles and methods, nursing and care. The main causes of disease combined with viscera deficiency, improper diet, emotional maladjustment, impairment of the kidney due to indulgence in sexual activities and exogenous factors. The pathogenesis of "Xiaoke disease" are complicated, the branch is the metabolic disorder of body fluid, while the root cause lies in the viscera disorder inside. Specifically, including stomach heat, spleen weak, heart fire, lung dryness and deficiency of kidney, and also concern with fire heat, stasis of blood, phlegm. Inner Canon of Yellow Emperor put forward a therapeutic principle, eliminate staleness. Zhang Zhongjing created special prescriptions for treating "Xiaoke". After that, physicians divided Xiaoke into three types, the treatments of Xiaoke are different, some emphasize Zang-fu organs; some emphasize Ying and yang, excess and deficiency; some emphasize Qi and blood, and others emphasize phlegm-dampness. A special syndrome differentiation in using TCM treat T2DM has been formed.Based on the patient from Professor Li Saimei during the period of 2014-2015,162 T2DM patients treated b are included. TCM group includes 103 patients. Based on the symptom scores, the effective rate was 71.84%. According to glycosylated hemoglobin, FBG,2h-PG, The effective rate was 66.99%. TCM+insulin group includes 29 patients. Based on the symptom scores, the effective rate was 55.17%. According to glycosylated hemoglobin, FBG,2h-PG, The effective rate was 86.21%. TCM+oral hypoglycemic drugs group includes 25 patients. Based on the symptom scores, the effective rate was 56.00%. According to glycosylated hemoglobin, FBG,2h-PG, The effective rate was 60.00%. According the prescriptions, finding out professor Li’s clinic characteristics of using TCM treatment to T2DM, "attaches great importance to the six-channel differentiation; use frigotherapy on the treatment of T2DM; strengthening spleen and tonifying kidney; keep the circulation of qi and blood"ConclusionThe results showed that TCM treatment can not only improve the subjective symptoms, but also effectively reduce the blood glucose and glycosylated hemoglobin.If the curative effect is still very poor after using insulin and oral hypoglycemic drugs, combine with TCM treatment can improve the therapeutic effects.
