| Objective:To observe and discuss the clinical effect of the decoction for the treatment of diabetic cerebral infarction.Methods:During March 2014 July 2016, in Affiliated Hospital of Yanbian University of traditional Chinese medicine and Western medicine combined with Department of Changchun University of traditional Chinese medicine and the Affiliated Hospital Institute of traditional Chinese medicine clinic of diabetic patients with cerebral infarction were randomly divided into for 36 cases of treatment group (Yiqi Jiedu Tongluo Decoction addition and subtraction treatment) and control group (acetyl salicylic acid tablets combined with citicoline sodium capsule in the treatment of) in 32 cases. There was no significant difference between the 2 groups in age, condition, duration of disease, gender and so on (P＞0.05). Yiqi Jiedu Tongluo Decoction (red ginseng 10g, Astragalus mongholicus 30g, Pinellia ternata 12g, Acorus gramineus Soland 15g, Cassia twig 12g, Honeysuckle 15g, Coptis chinensis 10g, Amomum tsao-ko 15g, Lycium chinensis 15g, Forsythia suspensa Vahl 15g, Chrysanthemum 12g, Ligusticum Chuanxiong 15g, Angelica 15g, Radix curcumae 15g, liquorice 5g) treated by modified, hyperactivity of liver Yang West ginseng to ginseng, Gastrodia elata, Uncaria; hemiplegia syndrome with earthworm, Achyranthes; language owes card is fluent and safflower, notoginseng; Kouyanwaixie scorpion, white monkshood, Bombyx Batryticatus. The observation group:automatic decoction machine, vacuum bag packaging, each bag of 200ml, one bag each time,30 minutes after breakfast and dinner for 30 minutes before taking; control group:acetylsalicylic acid tablets a 100 mg, once daily oral (breakfast 15 minutes after), citicoline capsule 0.2 g,3 times a day orally (). Course of treatment for 1 months, a total of 2 courses of treatment were observed. Main observation items:assessment of TCM syndrome curative effect, blood glucose testing (determination of fasting blood glucose, glycosylated hemoglobin, postprandial 2H blood sugar); serum lipids (triacylglycerol, low-density lipoprotein cholesterol, high density fat cholesterol), serum total cholesterol (TC). blood rheology study (whole blood viscosity, plasma viscosity, whole blood viscosity of Bigaoqie, whole blood low shear viscosity, plasma specific viscosity); brain CT or MRI detection.Results:1. The total effective rate of the treatment group and the control group were 86.1% and 1 respectively after treatment, and the treatment group was better than the control group (P< 0.05).2. The treatment group can effectively reduce the fasting FPG, HbAlc and 2hPG index (P< 0.05). There are significant differences (P< 0.05, P< 0.01), indicating that Yiqi Jiedu Tongluo Decoction in improving glucose metabolism than aspirin enteric coated tablets combined with citicoline capsule group has better curative effect by comparing the two groups.3. After treatment, the treatment group could effectively decrease CHO, TG (P< 0.05), and increase HDL-C (P< 0.01), LDL-C, and was not statistically significant (P> 0.05). The results showed that the Yiqi Jiedu Tongluo Decoction on blood lipid effect than aspirin enteric coated tablets combined with citicoline capsule group has a good effect.4. After the treatment, the treatment group in addition to the hematocrit showed no significant change (P> 0.05), platelet adhesion rate, fibrinogen and plasma viscosity, whole blood viscosity (high shear, low shear) were decreased significantly (P< 0.01, P< 0.05). Yiqi Jiedu Tongluo Decoction group changes in hemodynamic effect than aspirin enteric coated tablets combined with citicoline capsule group has a good effect.5. After treatment, the rate of syndrome of TCM in treatment group was significantly lower than that of the control group (P< 0.01). Yiqi Jiedu Tongluo Decoction of TCM syndrome score than aspirin enteric coated tablets combined with citicoline capsule group has better curative effect in reducing.Conclusion:The improvement of the glucose and lipid metabolism of the diabetic cerebral infarction by supplementing qi and removing toxin and dredging collaterals decoction has positive effect on the change of blood rheology and the decline of TCM syndrome integral. |
