| Objective:Observed the changes of skin tissue’s micro-structure in the related area of facial skin grafts, and then analyzing the significance of the outcome in the clinical application.Methods:56 patients from Department of Burn and Plastic Surgery, the First Affiliated Hospital of Guangxi Medical University, were selected to record the survival situation after the facial transplant surgery of middle-thick skin, full-thickness skin or subdermal vascular network skin graft. The related area(the donor site, facial skin graft surgery area, facial scar tissue and A small piece of normal facial skin) was used to do histological observation, after that, the significance of the outcome in the clinical application was analyzed.Results:50 (36 of moderate thickness skin,13 of full thickness skin and 1 of flap with dermal vascular network) of the skin grafts were totally survival on the 10-14th postoperative day,4(2 of moderate thickness skin,1 of full thickness skin and 1 of flap with dermal vascular network) were good in survival, 1 (full thickness skin) was qualified with small hematoma and 1 (moderate thickness skin) was needed operation again with superficial necrosis occur to the part of the grafts. According to the data, skin graft survival rate from high to low was the moderate thickness skin, full thickness skin and flap with dermal vascular network. The effect of looking in the short run was assessed. Flap with dermal vascular network was the best, and then the full thickness skin, and the moderate thickness skin at last. But there was no differences among these in the long run. Histological results were followed-up after years of the operation. It told us that the structure of the facial skin graft surgery area was different from the donor site, but was similar with normal facial skin. Conclusion:More importance and expect are given to plastic surgery on face and whether the cellulocutaneous flap can have the function of normal facial skin years post-operation or not. The changes of structure in the facial skin graft surgery area illustrated that the cellulocutaneous flap will have sweat glands and other glands the same as facial skin. It provides scientific laboratory theoretical basis for clinical treatment. |
