ObjectiveThe research is to evaluate the clinical efficacy of acupuncture combined with rehabilitation training on dysphagia after stroke, using surface electromyography(sEMG) to assess swallowing function in combination with Standardized Swallowing Assessment(SSA). What is more, as basis for further study, it was trying to explore mechanism of acupuncture treatment on dysphagia after stroke, and more scientific assessments.MethodsA total of 70 cases were included in the research, and they were in treatment group or control group. Using simple randomization method, according to the time of admission of the patients, each group was 35 patients. All patients were received treatment including clinical basic treatment of hypertension, diabetes and other diseases, and stroke conventional dialectical therapy. The treatment group received acupuncture combined with rehabilitation training, while the control group only received rehabilitation training. Acupuncture therapy in the research were included some acupuncture points as following:RN23, and two acupoints on left and right side of RN23 about 1 inch each, GB20(bilateral), GB12(bilateral), SJ17(bilateral), HN12, HN13, motor cortex under the 2/5 bottom side(bilateral). Rehabilitation training is based on condition of swallowing function of each patient, including indirect and direct training. Two groups of patients were treated for two weeks, Monday to Friday, once a day. Before and after treatment, two groups of patients were assessed with SSA and sEMG, and the occurrence of adverse events were recorded. The data was used SPSS 23.0 software package for data management and analysis.ResultsThis study was included 70 cases,35 in treatment group and 35 in control group. There was no death and loss of cases. Compared the effect of treatment group and control group before and after therapy with SSA and sEMG. Both the difference has statistically significance (P<0.05), indicating that the treatment in the two groups had a certain effect. Compared the effect of treatment after therapy between treatment group and control group with SSA and sEMG, and the total score difference between the two groups before and after treatment the difference has statistically significant(P<0.05), indicating that the effect of treatment group for dysphagia after stroke was better than the control group. It was also proved that acupuncture combined with rehabilitation training on dysphagia after stroke has better effect than that only with rehabilitation training.ConclusionIn this research, the effect of the treatment group for dysphagia after stroke was much better than that of control group, indicating that dysphagia after acupuncture combined with rehabilitation training has a good clinical significance. The results of sEMG with SSA showed that two kinds of evaluation methods for the disease assessment were both of positive significance, and worthy to be popularized in clinic. Mechanism of acupuncture on dysphagia after stroke might be local acupuncture strengthening the throat nerve and deep muscle stimulation. What is more, scalp acupuncture can activate swallowing movement area and surrounding tissue, enhancing domination of cortex to oral cavity, tongue, pharynx, and promoting function recovery. |
