Analysis Of The Causative Factors Of Ptosis Produced By Injection Rhinoplasty

Objective With the rapid development of professional plastic and cosmetic surgery, the public’s demand for becoming more beautiful by injection rhinoplasty is unceasing. And the number of the injection cosmetic surgery is increasing, along with the speeding-up of the marketization process. Because the appearance of the nose has a direct impact on the facial contours and overall look, and the nose shape of the Asians is lower, flatter and rounder than the Westerners. Injection rhinoplasty surgery has become a common micro-plastic surgery. Injection rhinoplasty surgery can cause irritation, skin necrosis, nodules, eye disorders such as blindness, and so on. Most scholars think that these were caused by nasal blood vessel damage or oppressed blood supply obstacles. Although these have gradually aroused people’s attention[1-3], the mechanism of the complications of the ptosis is not yet clear, and it lacks of a large number of literature reports, animal models and the experimental data. The purpose of this study is to analyze the patients that suffered from the ptosis induced by injection rhinoplasty in all aspects, study the possible pathogenic factors, infer the pathogenesis and summarize the treatment experience after giving the patients the corresponding comprehensive treatment.Materials and Methods In consideration of the clinical history, clinical manifestations and auxiliary examinations, 32 cases who had concurrent of ptosis induced by injection rhinoplasty in this study group, were given immediate comprehensive treatments, including local suction pressure, dissolving the enzyme, anticoagulation, improving the microcirculation, dilation blood vessels,anti-inflammatory, thrombolysis, hyperbaric oxygen, physiotherapy, reducing swelling,nutritional support, and so on.Results In the study group,18 patients with severe symptoms caused by ptosis improved to mild ptosis, 8 patients with symptoms of severe ptosis improved to moderate ptosis, 6 patients did not improve the conditions with blindness.Conclusions This study is to analyse comprehensively 32 patients of the ptosis produced by injection rhinoplasty, to detect injection areas and embolism related blood vessels with color doppler ultrasound instrument, and to compare affected side and healthy side of vascular diameter statistical. It is found that because of the complex structure of external nose and the upper eyelid anatomical, rich vascular network, vascular compression or embolism caused by injection filler material, ophthalmic artery blood supply area damage such as orbicularis oculi oculomotor nerve and muscle. In the end, neurogenic or muscle source sex ptosis occur. And, currently the types of injectable filler material are different; the surgery operation performers’ skills vary from high to low range; and the local vascular anatomy is different. These factors all can cause this complication.
