Objective:The aim of this study was toanalyze the effectiveness and long-term stability of the rapid maxillary expansion which was performed by means of using the palatal planting nail auxiliary screw expansionappliance inadults,compare the postoperativerelapse of adults rapid maxillary expansion withthat of juvenile in different periods, analyze the impact of age factor on the postoperative stability of palatal expansion, and to provide some reference and help for the design of clinical orthodontic treatment program. Methods:In order to investigate the effects of the expansion designed, the design of Experiment one, Curative effect analysis of planting nail auxiliary screw expansionand conventional screw expansion appliances in the treatment of maxillary arch expansion in adults,was designed.32 adults patients, aged between 18-25 years old, were randomly divided into two groups. Experimental group with 16 cases,underwent expansion with implant assisted screw expansion appliances. 16 cases in the control group, underwent expansion with conventional screw expansion appliances.Two groups of patients were measured before and after the expansion ofthe left and right maxillary first premolar, the first molar between the horizontal width, between the left and right maxillary first premolar, first molar and palatal intersection following 5mm horizontal width,the angle between the long axis of the bilateral maxillary first molar tooth were measured. Analysis of palatal width changes before and after, teeth alveolar tilt shift.In order to research the long-term postoperative stability of adults palatal expansion with the planting nail auxiliary screw expansion appliance, the design of Experiment two,a retrospective study of the postoperative stability of adults palatal expansion with auxiliary implants.Case selection and treatment method were the same with that of experimental group of experiment one.At different times preoperatively and postoperatively(0 months, 3 months, 6 months, 12 months), the horizontal width of 5mm below the intersection of midline palatine suture with palatine arches of attachment between bilateral maxillary first molars were measured. And the stability after palatal expansion was analyzed.In order to researchthe impact of age factor on the postoperative stability of palatal expansion,the design of Experiment three, Comparison of adults’ and juvenile’ postoperative stability of maxillary palate expansion. 16 cases of adult group,underwent expansion with implant assisted screw expansion appliance. 16 cases of juvenile group, underwent expansion with conventional screw expansion appliance.At different times preoperatively and postoperatively(3 months, 6 months, 12 months), the horizontal width of 5mm below the intersection of midline palatine suture with palatine arches of attachment between bilateral maxillary first molars were measured. This experiment compared postoperative recurrence of adults’ rapid maxillary expansion withthat of juvenile in same periods, analyzed the impact of age factor on the postoperative stability of rapid palatal expansion. Results:Experiment one:The results showed that there was no statistically significant difference between the experimental group and the control group in thehorizontal width changes of bilateral maxillary homonym teeth after expansion. The difference between the experimental group and the control group in the changes of horizontal width of 5mm below the intersection of midline palatine suture with palatine arches of attachment between bilateral maxillary homonym teeth,was statistically significant. In the experimental group and the control group, the angle changes of the bilateral maxillary first molars long axis were statistically significant.Experiment two : The results showed thatthree months during retention period,the horizontal width of 5mm below the intersection of midline palatine suture with palatine arches of attachment between bilateral maxillary first molars, is not changed after the expansion of the arch. However,comparing changes of six months after expansion with that of three months after the expansion,the difference was statistically significant. Comparing the horizontal width changes of six months’ after expansion with that ofone year after the expansion,the difference was statistically significant.Experiment three:The results showed that six months after expansion, the adult group recurrence proportion of palatal expansion of the total were significant differences with the juvenile group recurrence proportion of palatal expansion of the total. Twelve months after expansion, the adult group recurrence proportion of palatal expansion of the total were significant differences with that of the juvenile group. Conclusion:Experiment one:This study showed that the lateral width of the maxillary of adults can be effectively enlarged by the rapid expansion withplanting nail assisted screw expansion appliance, the palatal expansion is the mainly.Experiment two:This study showed that planting nail assisted young adults to expand the arch,expansion of palatal suture had a recurrence within one year.Experiment three:This study come to the conclusion that the recurrence of arch expansion after a year in an adult greater than the juvenile.
