| Objectives: Laparoscopic myomectomy fibroid impact of drilling on the use of residual and recurrent fibroids,comparison of patients with uterine fibroids laparoscopic and transabdominal myomectomy residual postoperative recurrence and pregnancy outcome.Methods:A retrospective analysis of patients under the July 2010 to July 2014 at the First Affiliated Hospital of Xinjiang Medical University hospital gynecological ward laparoscopic myomectomy in 420 cases,underwent transabdominal myomectomy patients with 291 cases,the use of laparoscopic surgery in patients undergoing all manner of using an electric drill fibroids, uterine fibroids electric drill model is Conkey KJ-301, patients with open surgical approach without using electric drill fibroids,compared two groups of patients with different surgical residual fibroids, postoperative recurrence and the effect on pregnancy, and analyzed using the residual rate of fibroids fibroids electric drill laparoscopic group of patients, the recurrence rate, pregnancy rate and postoperative recovery period.Results: Laparotomy patients excluded the largest myoma diameter(7.6 ± 3.0) cm,excluding the number of fibroids(5.6 ± 5.5),excluding the largest group of patients with laparoscopic myoma diameter(6.8 ± 2.0) cm, excluding the number of fibroids(2.4±2.1),laparotomy group were excluded largest myoma diameter greater than the number of removed fibroids laparoscopic patients,comparison of two sets of values respectively,the differences were statistically significant( P<0.05). Patients with residual fibroids laparoscopic group was 11.57%, the recurrence rate was 9.16%, postoperative pregnancy rate was 44.93%(31/69),laparotomy patients fibroids residual rate 10.49%, the recurrence rate was 10.14%,postoperative pregnancy rate was 64.29%(9/14),these three groups were compared values, P>0.05, the difference was not statistically significant. The number of uterine fibroids are the main factors affect recurrence( OR=2.605,95%CI 为1.182~4.602,P=0.0170).Conclusions:Laparoscopic and open myomectomy is the treatment of uterine fibroids effective and safe treatment,most uterine fibroids by laparoscopic surgery can be done this way surgery,laparoscopic myomectomy after surgery fibroids residual rate slightly higher than the laparotomy; short-term postoperative recurrence rate of fibroids almost similar,multiple fibroids are the major risk factors for recurrence of fibroids. Laparoscopic and open surgery after pregnancy close.Using laparoscopic myomectomy,minimally invasive, safe, rapid postoperative recovery, the advantages of small abdominal scar,and to preserve the uterus, maintain normal menstrual and reproductive function of women, although surgery fibroids clearance rate, however, due to some smaller fibroids before surgery gynecological examination can not be touched, and surgery naked eye can not see these small fibroids, caused some residual rate of fibroids. Secondly, due to surgery using an electric drill fibroids, when peeling fibroids, some debris fell into the pelvic fibroids,causing a certain degree of cultivation, improve postoperative recurrence rate of fibroids,therefore, the application of an electric drill fibroids need to further explore. |
