Objective:To explore the effects of maternal and child outcomes on pregnancy and uterine fibroids,the choice of delivery methods,and the feasibility of concurrent uterine fibroids resection for cesareanMethods:The observation group selected pregnant women who were found to have uterine fibroids after undergoing early pregnancy ultrasound examination atKowloonhospital from January 2013 to December 2019.During the study period,they underwent subsequent prenatal examinations and childbirth.At the same time,220 pregnant women without uterine fibroids were randomly selected as the control group.Inclusion criteria:1.Singleton,regular birth checkup during pregnancy.Gestational>28weeks 2.Exclusion criteria include:any history of uterine surgery,such as cesarean section,hysterectomy or hysteromyoma,any uterine deformity,adenomyosis(such as adenomyoma),any underlying disease,such as cerebrovascular,Cardiovascular or diabetes,and renal insufficiency.According to clinical data,statistical analysis of the percentage of pregnancy-associated uterine fibroids in various complications during pregnancy,childbirth and puerperium,and statistical analysis of pregnancy-combined uterine fibroids in vaginal delivery and cesarean delivery percentage.Specific comparisons are made between patients with a single uterine fibroid and patients with multiple uterine fibroids,between submucosal myomas,between myofibrary myomas and subserous fibroids,between patients with small uterine fibroids(diameter<5 cm),Differences between patients with large uterine fibroids(≥5 cm in diameter).In addition,according to the follow-up information,the clinical characteristics and short-term pregnancy outcomes were compared between myomectomy during cesarean section and non-myomectomy during cesarean section;in terms of uterine fibroid type,size,and The short-term pregnancy prognosis in terms of location compared hemoglobin,hematocrit,surgical time,length of hospital stay,and bleeding among uterine fibroidectomy patients during cesarean sectionResults:1.The comparison of the incidence of complications during pregnancy,childbirth,and puerperium during pregnancy with uterine fibroids(observation group)and pregnancy without uterine fibroids(control group).There were statistically significant differences i the complications of placenta,placenta previa and placental abruption(P<0.05).No statistical significance in blood transfusion,postpartum hemorrhage,and puerperal morbidity.2.A statistical analysis of the effects of pregnancy combined with uterine fibroids on perinatal infants showed that in the observation group and the control group,there was no statistically significant difference in neonatal malformations and premature babies,and statistical significance in neonatal asphyxia and low birth weight.3.The effect of pregnancy combined with uterine fibroids on different delivery methods The difference between transvaginal delivery and cesarean delivery in the observation and control groups was statistically significant(P<0.05).4.Compared with patients with single or multiple uterine fibroids(≥2),there were no statistics on threatened abortion,placenta previa,abdominal pain admission,abnormal fetal position,postpartum hemorrhage,spontaneous abortion,vaginal delivery and cesarean section.Statistically significant in terms of preterm birth(P<0.05).5.There are no statistically significant differences among the three groups of submucosal,intermuscular,and subserosal fibroids in threatened abortion,placenta previa,abdominal pain admission,postpartum hemorrhage,spontaneous abortion,premature delivery,vaginal delivery,and cesarean delivery There was a statistical difference in abnormal fetal position in the tumor group(P<0.05).6.Large fibroids(fibroid diameter≥5cm)are compared with small fibroids group(fibroid diameter<5cm)in threatened abortion,placenta previa,abdominal pain admission,abnormal fetal position,postpartum hemorrhage,spontaneous abortion,premature delivery,vaginal delivery,dissection Statistically significant in the incidence of uterine delivery(P<0.05).7.Comparison of clinical characteristics of patients with or without hysteromyoma during cesarean section,found no significant differences in maternal age,parity,pregnancy,gestational age,and fibroid size.8.There was no significant difference in hemoglobin concentration,hematocrit,and mean hospital stay between the group undergoing hysteromyoma resection during cesarean section and the group without hysteromyoma resection during cesarean section.However,when myomectomy was performed during cesarean section,the operation time increased(p<0.05).Multiple uterine fibroids and uterine fibroids located in the cervix and cornea are associated with increased hemoglobin and hematocrit,decreased bleeding rates,and prolonged surgical time.Conclusion:1.Pregnancy with uterine fibroids affects the incidence of complications during pregnancy,childbirth,puerperium and perinatal infants to some extent.2.Pregnancy with uterine fibroids affects the choice of delivery mode and increases the cesarean delivery rate.3.Simultaneous hysteromyoma resection during cesarean section in pregnant patients with uterine fibroids is safe and feasible.Compared to the myomectomy group during the cesarean section and the non-myomectomy group during the cesarean section,the average reductions in hemoglobin concentration,hematocrit,and mean hospital stay were similar,and there were no significant differences.However,the operation time was significantly longer when performing myomectomy during cesarean section(P<0.05).
