Objectives:By investigating the status of fasting、drinking forbidden and cognitive level,practice and explore the method to improve comfort of painless gastroscopy patients during waiting time, reduce the occurrence probability of adverse reactions as fasting for a long time, increase the patients comfort during waiting time, ensure medical safety and improve patient satisfaction on medical process.Methods:The research includes three aspects: the investigation on fasting 、 drinking forbidden of the painless gastroscopy patients, the experiment of the influence on the patients’ comfort of painless gastroscopy by shortening the time of drinking forbidden,the experiment of the influence on the patients’ comfort of painless gastroscopy by informing foreseeingly:1. The status investigation on fasting 、 drinking forbidden. This investigation selected 204 patients from the first hospital of Jilin University in Changchun as the respondents by convenience sampling method in February 2015, used self-designed questionnaire which includes two parts: the status of fasting、drinking forbidden and cognitive level. The questionnaire was given and recycled on the spot, the valid questionnaires was entered Epidata3.1, and analyzed by the SPSS17.0. The data was described by descriptives as mean, standard deviation, maximum, minimum,percentage and so on.2. The experiment of the influence on the patients’ comfort of painless gastroscopy by shortening the time of drinking forbidden. This experiment selected306 patients who accepted the painless gastroscopy and met the inclusion criteria from March 2015 to May in the first hospital of Jilin University in Changchun. The151 patients were in experimental group and the other 155 patients were in control group. Patients in the experimental group were in accordance with the conventional method of fasting and fed glucose solution of 200 ml which concentration is 5%, then drinking forbidden. Compare the physiological indexes and the comfort level of the two groups during the day of appointment booking, before the induction of anesthesia and the anesthesia recovery period. The valid questionnaires was entered Epidata3.1,and analyzed by the SPSS17.0. The data was analyzed by t-test, ANOVA, chi-square test and rank text.3. The experiment of the influence on the patients’ comfort of painless gastroscopy by informing foreseeingly. This experiment selected 328 patients who accepted the painless gastroscopy and met the inclusion criteria from May 2015 to July in the first hospital of Jilin University in Changchun. The 160 patients were in experimental group and the other 168 patients were in control group. Medical personnel combined with current endoscopic operation guide and patient appointment time to inform the patients in the experimental group that fasting time 、 drinking forbidden time and matters needing attention the day before examination in the form of telephone follow-up; the control group patients according to the conventional method to inform. Compare the the comfort level and satisfaction of the two groups during the day of appointment booking and before the induction of anesthesia. The valid questionnaires was entered Epidata3.1, and analyzed by the SPSS17.0. The data was analyzed by t-test, chi-square test and rank text.Results:1. The investigation has suggested, 204 painless gastroscopy patients’ fasting time is 10 ~ 25 h, the average time is(17.87±3.006) h, more than required 6 ~ 8h;actual drinking forbidden time is 6 ~ 25 h, the average time(16.75±3.589) h, more than 4h provisions. In the 204 patients, 52.9% patients usually taking oral medications,which 65.7% of patients due to accept painless gastroscopy to fasting and stop taking the oral medicine; in addition, 57.4% of the patients said before inspected without medical personnel to explain the fasting of purpose; 57.8% of the patients of fasting the importance of not understanding; 67.8% of patients think "the longer of the fasting and drinking forbidden time, the safer".2. In the experiment of the influence on the patients’ comfort of painless gastroscopy by shortening the time of drinking forbidden. Two groups of patients did not have vomiting and aspiration during the examination. The comparison results between the two groups of patients showed that the induction of anesthesia before the two groups of patients with systolic blood pressure, diastolic blood pressure, blood glucose value before the anesthesia compared, P<0.05, the difference has statistically significant, and in the experimental group, the blood pressure, blood sugar value level was higher than that of the control group. Compared with two groups of patients comfort level, P<0.05, the difference was statistically significant, the experimental group’s comfort level is higher than the control group.Comparisons within the groups showed that the patients in the experimental group had a relatively stable temperature, pulse, systolic blood pressure, diastolic blood pressure between the day of appointment booking and before anesthesia;compared to the value of blood glucose and the comfort level, P<0.05, the difference was statistically significant and anesthesia induced before the blood glucose value and comfort level was higher than the day of appointment booking. The patients of control group of patients showed that the contraction pressure, diastolic blood pressure, blood glucose and comfort levels, P < 0.05, the difference was statistically significant and the blood pressure, blood glucose level and comfort level was lower than the day of appointment booking.3. The experiment of the influence on the patients’ comfort of painless gastroscopy by informing foreseeingly showed that before the anesthesia, the patients in the two groups of hunger, thirst, fatigue, feeble, weakness compared P< 0.05, the difference was statistically significant, and the comfort level of the patients in the experimental group was higher than that of the control group; compared two groups of patients satisfaction score of medical personnel and medical process, P < 0.05, the difference was statistically significant and satisfaction of patients in experimental group was higher than control group.Conclusions:1. The painless gastroscopy patients’ fasting and drinking forbidden time was more than the set time. The long time of fasting and drinking forbidden will not only affect the physiological index in patients with stable, but also increase the adverse reactions and reduce the comfort level during the waiting time.2. The medical personnel’s is not sufficient, for different patients in the mission are "one size fits all" phenomenon. Emphasizing the impact of lacking fasting and drinking forbidden time, ignoring the adverse reactions caused by the long fasting and drinking forbidden time. And most patients of painless gastroscopy grasp the lack of related knowledge about fasting and drinking forbidden. Some even according to their subjective judgments and their own to extend the fasting and drinking forbidden time,resulting in patients appear hunger, thirst, anxiety and other negative feelings and affected their comfort level during waiting time.3. Shorten the drinking forbidden time will not affect the painless gastroscopy patients’ examination results, can’t increase the risk of complications during the examination, but also help to maintain the normal metabolism of the body level,ensure stability of physiological indexes, increasing the value of blood glucose concentration, improve the comfort level during waiting time.4. Informing foreseeingly can effectively improve the patients’ comfort level and satisfaction during waiting time.
