| Research background: The fracture and dislocation around carpal bone takes up about 10% in wrist injury, especially the scaphoid perilunate dislocation, accounting for about 3%-5%[1]. Dorsal type fracture of scaphoid perilunate dislocation is the most common type in such injuries, and the wrist 3D CT can clearly reconstruct the relationship of month bone, capitate bone, scaphoid bone and the third metacarpus bone. By now,there is no unified standard of the treatment of the scaphoid perilunate dislocation,which cause the different curative effect.With the delayed treatment or wrong treatment, the latter often lead to proximal row carpectomy [2][3].So it has a high morbidity.Objective:To investigate the function of external fixator with hollow screw and Kirschner wire in the treatment of the scaphoid perilunate dislocation.Methods: 18 patients in clinic were divided into two groups, wrist external fixator were used in 9 cases, with wrist open reduction, hollow screw fix the scaphoid fracture, Kirschner wire fix the joint of capitate bone and semilunar bone, repair intercarpal ligament; 9 cases with hollow screw fix the scaphoid fracture, temporary Kirschner wire fix the joint of capitate bone and semilunar bone,repair the intercarpal ligaments and plaster external fixation after restoration. All patients were followed up for 1-2 years, The statistical analysis are used in wrist flexion and extension, ulnar deviation, radial deviation and the Cooney point table.Results: Through t test,There are significant differences beteen the two groups in wrist flexion and extension and Cooney total point, P<0.05. There are no difference beteen ulnar deviation, radial deviation.Conclution: The external fixator ã€hollow screw and Kirschner wire play an important role in the treatment of scaphoid perilunate dislocation, as it may facilitate the early ligament repair and the recovery of wrist joint function,with Satisfactory effect. |
