Objective:This study explore the feasibility and effectiveness of the treatment of inguinal incarcerated hernia by the laparoscopic hernia repair before the peritoneal(laparoscoplc trans-abdominal preperitoneal hernia repair, TAPP).Methods:retrospective study of Qinghai university affiliated hospital between Oct 2012 to Apr 2014 of 65 patients with inguinal incarcerated hernia, According to admission order number, use the random Numbers table extraction method of grouping:20 cases of A group(laparoscopic inguinal incarcerated hernia reduction + TAPP); 21 cases of B group(line inguinal incarcerated hernia orif + OR); 24 cases of C group(line inguinal incarcerated hernia orif + HL),to analyze three groups. Observe and compare the following indexes of three groups of patients: including operation time, intraoperative blood loss, postoperative gastrointestinal function recovery time, analgesia drug use cases, to participate in normal activity time and postoperative complications were analyzed.Results:(1) the surgery time of group A long operation time in the group B and C group(P < 0.05);(2) A group of intraoperative blood loss, hospital stay, ambulation time, gastrointestinal function recovery time and analgesic drug use cases, compared with group B, C, differences were statistically significant(P < 0.05);(3) three sets of postoperative incision scleroma, subcutaneous hematoma, scrotal swelling index comparison, there was no statistically significant difference(P > 0.05);(4) A set of postoperative recurrence rate is lower than B and C group, the difference was statistically significant(P < 0. 05);B recurrence rate lower than C group, the difference was statistically significant(P < 0. 05).Conclusion: the laparoscoplc trans-abdominal preperitoneal hernia repair(TAPP) can fully explore the abdominal cavity, avoid omissions of necrotic bowel, has small trauma, rapid recovery, short hospital stay and recurrence rate, TAPP is a kind of effective treatment of inguinal incarcerated hernia and feasible operation, but according to the actual circumstance of patients timely clinical application.
