| Objective: To summarize Professor Ma Hongbin’s clinical experience of treatment of gouty arthritis; to evaluate the clinical efficacy of Qingrepaidu Capsule plus indomethacin, Qingrepaidu Capsule, indomethacin treatment of acute gouty arthritis, by randomized controlled clinical observation. Methods:(1) Follow the professor, organize and summarize the clinical experience of the supervisor Professor Ma Hongbin’s treatment of gouty arthritis.(2) This study included 90 patients with acute gouty arthritis, randomly divided into Chinese patent medicine- western medicine, Chinese patent medicine, and western medicine group by the SPSS 17.0 software, by comparing the VAS variation of pre-treatment, after a course of treatment, after 2 cycles, and the uric acid variation of pre-treatment, after a course of treatment, after 2 cycles, to evaluate the clinical outcomes of Qingrepaidu Capsule in treatment of patients with acute gout. Results:(1) The supervisor Professor Ma Hongbin consider that the causes of gouty arthritis cover an unhealthy diet, body fat, living carelessly, irregular life, its pathogenesis key include spleen deficiency, unable to distinguish between the clear and the muddy, hot toxic infestation. Toxic heat, phlegm, blood stasis are the main pathological product, in the acute phase, damp-heat accumulation were very common, which are equivalents of Chinese medicine "Re Bi" category. The treatment advocated combining differentiation with disease, which brought about a striking effect in the clinical application.(2) Comparison of clinical efficacy, in the Chinese patent medicine- western medicine group, 8 cases were cured, 17 cases were improved, and 3cases did not improve, the total efficiency is 89.3%; in the western medicine group, 2 cases were cured, 21 cases were improved, 7 cases did not improve, the overall efficiency reached 76.7 %; in the Chinese medicine group, 0 cases were cured, 24 cases were improved, 6 cases did not improve, the overall efficiency reached 80%.Through the statistical treatment, the curative effects of comparative differences in three groups have very remarkable significance(P<0.05).(3) Comparison of VAS variation after a course of treatment : by comparing the VAS variation of pre-treatment, after a course of treatment in three groups of patients, through the statistical treatment, before treatment and after treatment, there are very statistical significance in Chinese patent medicine- western medicine group(P<0.05); Comparison of VAS variation after a course of treatment among the three groups after a course of treatment, the statistical treatment of Chinese patent medicine- western medicine group and western medicine group reached a very statistical significance(P<0.05); The statistical treatment of Chinese patent medicine- western medicine group and Chinese medicine group reached a very statistical significance(P<0.05); The statistical treatment of proprietary Chinese medicine group and western medicine group reached a very statistical significance(P<0.05).(4) Comparison of VAS variation after two courses of treatment : by comparing the VAS variation of pre-treatment, after two courses of treatment in three groups of patients, through the statistical treatment, before treatment and two courses of treatment, there are very statistical significance in Chinese patent medicine- western medicine group(P<0.05); Comparison of VAS variation after two courses of treatment among the three groups, the statistical treatment of Chinese patent medicine- western medicine group and western medicine group reached a very statistical significance P<0.05); The statistical treatment of Chinese patent medicine- western medicine group and Chinese medicine group reached a very statistical significance(P<0.05); The statistical treatment of proprietary Chinese medicine group and western medicine group had no statistical significance(P>0.05).(5) Comparison of blood uric acid variation after two courses of treatment : by comparing the blood uric acid variation of pre-treatment, after two courses of treatment in three groups of patients, through the statistical treatment, before treatment and two courses of treatment, there are very statistical significance in Chinese patent medicine- western medicine group(P<0.05); Comparison of blood uric acid variation after two courses of treatment among the three groups, the statistical treatment of Chinese patent medicine-western medicine group and western medicine group reached a very statistical significance( P<0.05); the statistical treatment of Chinese patent medicine- western medicine group and Chinesemedicine group reached a very statistical significance(P<0.05); the statistical treatment of proprietary Chinese medicine group and western medicine group had no statistical significance(P>0.05).(6) Correlation analysis: between the uric acid of patients with acute gouty arthritis and the patients’ pain intensity(VAS integration before treatment), Pearson correlation coefficient is r= 0.646, P<0.001. Conclusion:(1) The supervisor’s experience is consistent with clinical practice, disease and syndrome, prescription and conditions, which improve the clinical efficacy.(2) Qingrepaidu Capsule plus indomethacin, Qingrepaidu Capsule, indomethacin treatment of acute gouty arthritis can significantly improve the patient’s joint pain, and lower blood uric acid.(3) After a course of treatment, in terms of relieving patients’ joint pain, Qingrepaidu Capsule plus indomethacin, is significantly better than Qingrepaidu Capsule and indomethacin, indomethacin is better than Qingrepaidu Capsule.(4) After two courses of treatment, in terms of relieving patients’ joint pain, Qingrepaidu Capsule plus indomethacin is significantly better than Qingrepaidu Capsule(5) After two courses of treatment, in terms of lowering patients’ uric acic, Qingrepaidu Capsule plus indomethacin is significantly better than Qingrepaidu Capsule and indomethacin.(6) The VAS score of patients with acute gouty arthritis and their uric acid make positive linear correlation. |
