ObjectivesThe purposes were to study the changes in parameters of Microcirculatory blood vessels(MBV),when exposed to High Frequency Ultrasound(HFU),to evaluate evaluate the therapeutic effects and differences of 595 nm pulsed dye laser(PDL) upon different types of hemangiomapatients. to analyze 595 nm PDL’s damages to skin and its stability by observing pathological sections with HE staining and PAS staining resulted from different amount of energy exposure, and to compare.Methods1. Remove the hair on back of 30 KM mice,and each is divided into six laser fields. According to the observation time,the mice are divided into five testing groups, with 6 in each. Every group is further divided into two subgroups of three. They are all exposed to 595 nm pulsed dye, divided into two subgroups, each subgroup of three, to irradiate in 595 nm pulsed dye laser in high(11 J/cm2), medium(8 J/cm2),and low(6 J/cm2) of different laser energy(7 mm spot size, pulse width 1.5 ms), one subgroup uses erythromycin ointment with wounds and the other one uses the other one use benzalkonium bromide solution. Pathological sections are made from all the experimental specimen. After HE and PAS staining, they are observed under light microscope on the morphological changes of skin superficial ultramicro-structure and damage depth, evaluating the different energy stability of 595 nm pulsed dye laser, comparing the effect of wounds repair by smearing erythromycin ointment to create a moist environment,or conventional benzalkonium bromide solution.2.Use 595 nm pulsed dye laser irradiation for the treatment of patients with hemangioma, and compare the data before and after treatment with HFU. The patients’ recoveries are observed.Evaluate the different effects of the treatment upon patients with different types of hemangioma.Results1. General observation of KM mice skin after laser irradiation: Bleeding and crusting of the skin gradually aggravated with the increase of energy value. Temperatures of skin, after laser irradiation of Group A,significantly increased than before irradiation.The wounds healed more quickly smearing erythromycin ointmentthan with benzalkonium bromide solution.2. Histopathology after laser irradiation(HE and PAS) observations: In medium-energy group with erythromycin(8 J/cm2),after irradiation 0 h, 24 h, 72 h, 1 week, 1 month, the depth of damage decreased[(0.30 + 0.04) mm-(0.11 + 0.02) mm].In high-energy group with erythromycin(11 J /cm2) after irradiation 0 h, 24 h, 72 h, 1 week, 1 month, the depth of damage decreased(0.43 + 0.05) mm-(0.20 + 0.02) mm;In medium-energy group with benzalkonium bromide solution(8 J/cm2),after irradiation 0 h, 24 h, 72 h, 1 week, 1 month, the depth of damage decreased(0.30 + 0.04) mm-(0.14 + 0.02) mm, In high-energy group with benzalkonium bromide solution(11 J /cm2),after irradiation 0 h, 24 h, 72 h, 1 week, 1 month, the depth of damage decreased(0.43 + 0.05) mm-(0.26 + 0.03) mm(P < 0.05,difference between groups had statistical significance).The injury increased significantly in superficial tissue after laser treatment when the energy value went up. It was not obvious in low energy groups. Inflammation could be found around the damage areas, also the disorder of tissues. The shape of stratumbasaleliked saw tooth, with regular collapse appeared. The depth of defect increased with the increase of energy.3. Described the bar chart of mean changes according to the damage depth from pathological sections. In transversal comparison of wound recoveries with 595 nm pulsed dye laser, the groups smearing erythromycin damaged lighter than that with benzalkonium bromide solution. The energy(8 j/cm2) is the most obvious.4. Observed patients with different types of hemangioma after 595 nm dye laser treatment, using high frequency ultrasound. The Vmax reduced mean in each groups of different ages showed a decline tendency(P<0.05, have statistical significance).In transversal contrasting of decrease percentage in three types of hemangioma, the nevus flammeus groups and the strawberry hemangioma groups decreased more obviously than the cavernous hemangioma groups(P < 0.05, have statistical significance). No obvious adverse effects occurred on the most patients.It showed the coagulation of the laser for small blood vessels, and the effectiveness of the treatment.Conclusion1. It shows good stability in the treatment of superficial hemangioma by 595 nm pulsed dye laser.2. It shows a positive correlation between the different energy of 595 nm pulsed dye laser and the damage of treatment.3. It can be better for the repair of superficial tissue by using erythromycin ointment to keep the wound moist.4. It has important guiding significance in the treatment of superficial skin hemangioma with 595 nm pulsed dye laser combining of HFU.
