Objective:To compare the clinical effect of discectomy combined the Dynesys and simple discectomy treatment of the patients with lumbar disc herniation,and provide the basis for future surgical planning.Methods: A retrospective analysis about 50 cases of 20-50 years old patients with lumbar disc herniation between October 2011 and December 2012 in Hospital Affilated to Shan Dong University of Traditional Chinese Medicine,they were divided into two groups,25 cases in each group.The group A was treated with Intervertebral fenestration discectomy combined the dynesys,and the group B for Intervertebral fenestration discectomy. each patient with single segment of disc herniation, generally with lumbodynia or lower limb radicular pain. The postoperative follow-up time was at least 1 year,with an average of 20 months. Surgical effect was assessed by visual analogue scale(VAS),the Japanese Orthopaedic Association(JOA)score, the recovery rate and Oswestry disability index(ODI).Results: During follow-up, group B had a case of lumbar instability and one case of recrudescent lumbar disc herniation,while the group A had no case of them.the incidence of instability and recurrence in group A were significantly lower than those in group B(P<0.05).Both the two groups achieved Satisfactory clinical effect, postoperative Oswestry disability index(ODI), VAS score and the Japanese Orthopaedic Association score were significantly improved compared with the preoperative data, there was significant difference(P<0.05). At the final follow-up group A was better than group B assessed by Oswestry Disability Index(ODI), visual analogue scale(VAS)score and the Japanese Orthopaedic Association(JOA) score,but there was no statistical significance between two groups(P>0.05).Conclusion: Both the two treatment methods were effective, the clinical results of group A was better than group B.Dynesys was useful to maintain the stabilization of the spine and helpful to prevent the degeneration of the instrumented segments and adjacent segments. |
