| Objectives:This research is 1) to evaluate the incidence rate of suicidal ideation in acute phase ischemic stroke patients, 2) to explore the influence factors of suicidal ideation in acute phase ischemic stroke patients, 3) to analyse the adverse outcomes caused by suicidal ideation in acute phase ischemic stroke patients, 4) to provide scientific theoretical basis for clinical staff making nursing intervention and improving the life quality of stroke patients. Object and Methods:Adopt the method of cross-sectional study and prospective cohort study. From July 2013 to December 2013, a total of 271 consecutive patients with acute phase ischemic stroke were recruited in a third rate hospital neurology clinic, Henan province, China, to explore the incidence rate of suicidal ideation in acute phase ischemic stroke patients and influence factors and investigate the adverse outcomes of suicidal ideation in stroke patients after one year follow-up. The content of investigation questionare includes :(1) the patient’s general demography data: age, sex, marital status, level of education, residence, family average income.(2) disease clinical data: pre-stroke depression, comorbidities, post-stroke depression, lesion location, confidence in disease treatment, history of stroke and sleeping condition.(3) BSI(Beck Scale for Suicide Ideation) is to evaluate the intensity of suicidal ideation in ischemic stroke patients.(4) SDS(Self-rating Depressive Scale) is to evaluate the status of depression in stroke patients.(5) SSRS(Social Support Rating Scale) is to understand the condition of social support.(6) FAI(Family Apgar Index) is to know the status of family care.(7) TCSQ(Trait Coping Style Questionnaire) is to evaluate the difference between negative coping and positive coping style.The software of Epi Data 3.1 is used for data entry and SPSS 13.0 software for statistical analysis. Univariate analyses were performed by using chi-square, t test, multivariate analysis using Logistic regression analysis. Results:1. Demographic data of 271 patients with acute phase ischemic stroke presented that mean age was 64.5 years(SD 12.2) old with male 146 cases(53.9%) and female 125 cases(46.1%); 228 patients(84.1%) got married and 43 patients(15.9%) were unmarried; 225 patients(83.0%) had primary and junior high school education, 35 patients(12.9%) had high school education and 11 cases(4.1%) had college and above education; 125 cases(46.1%) were living in urban, while 146 cases(53.9%) were in rural; family average income of 141 cases(52.0%) were less than 1000 yuan/month, 109 cases(40.2%) had family average income ranging from 1000 to 2000 yuan/month and the rest 21 cases(7.7%) had more than 2000 yuan/month. 29 cases(10.7%) patients had suicidal ideation in 271 acute phase ischemic stroke patients.2. With suicidal ideation and without suicidal ideation group were different statistical in the residence(P=0.012), pre-stroke depression(P<0.001), complication with diabetes(P=0.033), NIHSS score(P<0.001), post depression(P<0.001), disease treatment confidence(P<0.001), trait coping style(P<0.001), P<0.05.3. Binary logistic analysis showed that the factors influencing suicidal ideation in acute phase ischemic stroke patients were living in rural region(P=0.010, OR=4.658), pre-stroke depression(P<0.001, OR=6.908), having no confidence in disease treatment(P=0.019, OR=9.393), high NIHSS score(P=0.001, OR=1.127), post depression(P<0.001, OR=8.416).4. After one year follow-up for the study objects, we found that with suicidal ideation and without suicidal ideation group had statistical significance in recurrent(c2 =7.086, P= 0.029). Conclusion:1. Acute phase ischemic stroke patients suicidal ideation rate is 10.7%, which is consistent with foreign studies repoting the incidence rate of suicidal ideation.2. Living in rural region, pre-stroke depression, having no confidence in disease treatment, high NIHSS score, post depression are the main factors of acute phase ischemic stroke patients suicidal ideation.3. Ischemic stroke patients in acute phase with suicidal ideation will increase the possibility of recurrent of stroke and influence the rehabilitation and the quality of life of stroke patients. |
