Objective:Analyze MRCP imaging manifestation in the obstructive bile duct diseases, and explore the role of MRCP in the obstructive bile duct diseases.Methods:Of the 51 patients with biliary obstructive diseases,30 patients(team A, n=30)have pathologically proven benign after operation,21 patients(team B, n=21)have proven malignant. All the patients have undergone MRCP examination with Siemens 3.0 T high field strength MRI instrument, and the original data was processed with the 3D image reconstruction. Combining the 3D MRCP images with the clinical pathological diagnosis, analysis of the imaging features of biliary tree expansion and some indirect imaging findings of biliary tree expansion, identify if the lesion is benign or malignant.Results:1.According to the expansion of bile duct, grade one is divided into team A (mild 20%, moderate 60%, severe 20%) and team B(moderate 14%, severe 86%):2. Intrahepatic bile ducts dilation range team A and team B respectively, (<2/3 count for 73%, full dilation 27%), (<2/3 count for 24%, full dilation 76%):3. Grades of intrahepatic bile ducts dilation:for team A grade 2 in 2 cases (7%), grade 3 in 16 cases (53%), for team B, grade 4 in 11 cases (37%), grade 5 in 1 case (3%).4. Shapes of intrahepatic bile ducts:for team A, soft vines 10%, dry tree shape 90%, for team B soft vines 76%, dry tree shape 24%.5.Degree of extrahepatic bile duct dilation:for team A, mild, moderate and severe respectively, accounting for 30%,50% and 30%, for team B,24%,9%,67%, respectively.6.The shape of Obstruction parts:pour the glass shape:for team A, pour glass shape (57%), truncation shape (20%), cone shape (3.5%), rat caudate shape(7%), and partial mind narrow (13%), for team B,5%,48%,28%,14%,5%, respectively. The 6 indexes above in comparing team A and team B have a statistical significance.7.The relative degree of bile duct in team A and team B are 1.83±0.68,1.43±0.21 respectively, and have a statistical significance(p<0.05).Conclusion:MRCP can display the dilation of bile ducts, bile ducts and the site of biliary tree stricture, providing a clinical significance in differentiating benign and malignant obstructive bile duct diseases.
