Objective: To investigate the diagnostic value of magnetic resonance cholangiopancreatography(MRCP)combined with magnetic resonance coronal imaging in biliary obstruction.Methods: 70 patients with biliary obstruction confirmed by pathology from October 2015 to August 2020 were selected as the research objects.All patients underwent MRCP and MRI coronal imaging(plain scan and enhanced scan)before operation,and the imaging manifestations of MRCP and MRI coronal imaging of biliary obstruction were analyzed;the accuracy of MRCP,MRI coronal imaging and their combination in localization diagnosis of biliary obstruction was compared.To compare the accuracy of MRCP,MRI coronal imaging and their combination in the qualitative diagnosis of biliary obstruction;to compare the effectiveness of MRCP,MRI coronal imaging and their combination in the qualitative diagnosis of malignant biliary obstruction.Results:(1)The accuracy rates of MRCP,MRI coronal imaging and their combination in the localization diagnosis of biliary obstruction were 90%,88.6% and95.7% respectively,and there was no significant difference among the three groups(P >0.05).(2)The accuracy of MRCP,MRI coronal imaging and their combination in the qualitative diagnosis of biliary obstruction were 71.4%,82.9% and 94.3%,respectively.The accuracy of combined examination was higher than that of MRCP and MRI coronal imaging alone,and the difference was statistically significant(P < 0.05).There was no significant difference in the accuracy of qualitative diagnosis of benign biliary obstruction(P > 0.05).The accuracy of qualitative diagnosis of malignant biliary obstruction was 58.7%,78.3% and 93.5%,respectively,and the accuracy rate of MRI coronal examination was higher than that of MRCP,and the accuracy rate of combined examination was higher than that of MRCP and MRI coronal examination alone(P <0.05).(3)In the qualitative diagnosis of malignant obstructive disease,the sensitivity of MRCP,MRI coronal imaging and their combined diagnosis were 58.7%,78.3 %and93.5%,respectively.The sensitivity of combined examination was higher than that of MRCP and MRI coronal imaging alone,and the difference was statistically significant(P < 0.01).The specificity of MRCP,MRI coronal imaging and their combined diagnosis were 95.8%,91.7% and 95.8%,respectively Significant statistical significance(P > 0.05).Conclusion: Both MPCP and MRI have high diagnostic accuracy in localization of biliary obstruction.In the qualitative diagnosis of biliary obstruction,the accuracy of MRI coronal imaging in the qualitative diagnosis of malignant biliary obstruction is better than MRCP coronal imaging,and the combination of the two can significantly improve the qualitative diagnosis of biliary obstruction.In the qualitative diagnosis of malignant biliary obstruction disease,MRCP Combined with MRI coronal imaging has obvious advantages in diagnostic sensitivity compared with single examination method.In conclusion,In conclusion,MRCP Combined with MRI coronal imaging has high diagnostic value for the etiology of obstructive jaundice,which provides a reference for the selection of clinical treatment,and is worthy of wide application in the diagnosis of biliary system diseases.
