ObjectivesTo summarize the clinical features, diagnosis and treatment experiences for unexpected gallbladder carcinoma(UGC) during or after laparoscopic cholecystectomy (LC), to discuss the procedures that might promote radical resection rate and long-term efficacy.MethodsThe clinical data of64patients with UGC in LC were retrospectively analysed.ResultsAmong the64cases,28cases received LC and36cases received additional radical resection besides LC, all the64cases were obtained for2~132months of follow-up. The difference in post-operative survival rate between the group that received LC and the group that received additional radical resection besides LC were statistically significant (P<0.05). When discussed according to the TNM staging system, the post-operative survival rate between the two groups for pTis+pTla grade cases was not statistically significant (P>0.05), but the differences for pTlb, pT2and pT3grade cases were statistically significant (P<0.05).ConclusionsLaying great stress on high risk factors and performing anatomical specimen examination and intraoperative rapid pathologic examination could improve radical resection rate. Radical cure could be achieved for pTis and pTia stage UGC by LC., for pTlb grade UGC, determination of surgical procedures needs further discussion. For UGC staging pT2or higher grade, radical surgery should be added for improvement of long-term efficacy.
