Introduction: Acute cholecystitis, which is also known as gallbladder inflammation, commonly occurs alongside gallstones.Laparoscopic cholecystectomy is the preferred surgical treatment for symptomatic gallbladder disease following initial conservative management, and it can be performed either early or later. In cases of acute biliary pancreatitis, cholecystectomy is necessary to prevent further biliary events. However, the ideal timing for this procedure is still a matter of debate, with the British Society of Gastroenterology recommending that it be done either during the same hospital admission or within two weeks of discharge. It is worth noting that the definition of "early" may differ depending on the guidelines being followed. Methods: The Department of General Surgery at Southwest Medical University enrolled 124 patients who met the criteria for acute biliary pancreatitis after obtaining approval from the hospital’s ethical review committee.Patients were screened to ensure they met the inclusion and exclusion criteria and gave written informed consent before participating in the study. The patients were randomly assigned to two groups,Group A and Group B,through a lottery system.Group A received early cholecystectomy, while Group B underwent delayed cholecystectomy.Perioperative complications and patient outcomes were closely monitored and evaluated one week after surgery. Results: The study included a total of 124 individuals, with an overall mean age of 44.5 ± 9.7months for both the early and delayed groups. The study also found a statistically significant mean difference(p-value <0.05)in terms of total hospital stay between the two groups.Additionally, the mean difference in operative time(in minutes)and total hospital stay between the early and delayed groups was analyzed. Conclusion: To summarize our findings, this study concludes that for patients with mild to moderate acute biliary pancreatitis, undergoing laparoscopic cholecystectomy early in the hospital admission period can lead to a reduction in both operative time and total hospital stay.
