BackgroundBecause of an increasing number of complex bone and joint traumas and the new development of the appraisal standard of body traumas degree, each hospital faces a growing number of treatment and forensic work in the areas of complex bone and joint traumas; At the same time, because of the complex bone and joint traumas are easily missed or over-diagnosis, so that radiologists face increasing pressure and responsibility. It is important to learn how to utilize various imaging examinations and make a objective suggestion for radiologists, which can contribute to the diagnosis and management on complex bone and joint traumas. Lots of studies about the value of MSCT and MRI in the diagnosis on complex bone and joint traumas had been conducted, and many rich experience has been achieved in these studies, but it is insufficient to compare the value of MSCT and MRI, combined or separate, in the diagnosis on complex bone and joint traumas.ObjectiveThe aim of this study was to examine the detection rate of MSCT combine with MRI and MSCT and MRI in the diagnosis on complex bone and joint traumas, and investigate how to achieve complementary advantages In the MSCT and MRI applications to provide more accurate and reliable image evidences for clinical treatment and judicial identification.MethodsThis is a randomized double-blind contrast test. We collected the clinical and imaging information of 43 patients with complex bone and joint traumas, which had been authenticated in the procedure of surgical operation, or a result of clinical follow-up. All imaging information were observed separately by three radiologists, using MSCT and MRI, combined or separately. Observations including the number of fractures, bone fragments and bone contusions, the injury of cartilages and ligament, were expressed as incidence rate, and all that were analyzed using SPSS version 17.0ResultsThere were 167 fractures found in 45 anatomic sites had been authenticated in the procedure of surgical operation, or a result of clinical follow-up. In MSCT group,36 fractures had been found in 142 anatomic sites, and the response rate of finding of anatomic site and fracture were 80.000% and 85.03% respectively. In MRI group, there were 35 fractures in 136 anatomic sites,75.56% and 81.44% respectively. In MSCT combined with MRI group, significantly, there were 44 fractures in 162 anatomic sites, 97.78% and 97.01% respectively(p<0.05). As we found, the combination of MSCT and MRI is more powerful in sensitivity than MSCT and MRI separately, especially, in insidious fracture and dislocated bone fragments(p<0.05). Compared with MSCT and MRI has an outstanding superiority in injury of ligament and cartilage, bone marrow edema, bone contusion and meniscus tear.ConclusionThe combination of MSCT and MRI has obviously improved on the detection ability of insidious fracture and dislocated bone fragments, and is more efficient,perfect in the diagnosis of complex bone and joint injury traumas, and can provide more definitive and elaborate information for clinical treatment and judicial identification. For the diagnosis of complex bone and joint traumas, it is essential to utilize various iconography equipment synthetically, such as X-ray, CT, MSCT and MRI,and it is necessary to apply the method of comparative imaging and follow the safe and effective,economic and simple,easonable and orderly principles.
