| Objective:The objective of the research is to explore the relationship between theseindexes and HHD and the clinic significance and then provide a basichelp for the early diagnosis and treatment of the disease by measuring thelevel of markers of myocardial injury such as FT3, BNP, cTnI, Mb,cardiac enzymes as well as other biochemical indexes and analying thechange of them.Method:Collected426patients diagnosed of hyperthyroidism from January2012to December2014treatments in China-Japan Union Hospital of JilinUniversity, which include132cases of HHD (46cases were male and86were females,ages from33to82years old) and294cases of simplehyperthyroidism (113cases were male and181were females, ages from13to79years old). Measured the concentration of TSH, FT3, FT4, cTnI,My, NT-proBNP, AST, CK, CK-MB, LDH, α-HBDH, TP, ALB, PA, TG,TC, HDL-C, LDL-C and GLU of the two groups and analyzedstatistically.Used SPSS17.0statistical package to do statistical analysis: All datalines normal inspection, Adopted T-test to compare the difference between groups, P<0.05indicates a significant difference, with statisticalsignificance; P>0.05isn’t statistically significant.Results:(1)The incidence rate of Cardiac injury in Hyperthyroidism was30.98%.(2)There are statistics differences between two age groups and the courseof the diseases(p<0.05). In this set of data, the incidences rate of atrialfibrillation, heart enlargement, heart failure were62.9%,47.0%and28.0%. And in all types of hyperthyroid disease, it is Graves Disease thatshares the highest proportion of HHD(73.5%).(3)There are differences between two groups of FT3, cTnI, Mb, CK-MB(p<0.05) and more obvious of NT-proBNP(P<0.01), but no sign-ificant difference of FT4, TSH and HBDH in blood(P>0.05).(4)Serum TP, ALB, PA, TC, HDL-C and LDL-C of HHD group aresignificantly lower than the control group, while the GLU wassignificantly higher(P<0.01).(5)NT-proBNP can be used as one of diagnostic indicators suitable forhyperthyroid heart disease, When NT-proBNP level of137.19pg/mL wasproposed as a cut off point, the sensitivity, specificity, positive predictivevalue and negative predictive value were95.45%,90.48%,87.5%and97.79%respectively for the diagnosis of HHD. Conclusion:The incidence of HHD is high, and it is related to the ages of patients aswell as the courses of hyperthyroidism. Markers of myocardial injury inpatients with HHD such as FT3, cTnI, Mb and CK-MB were higher thanother two groups, which is crucial for diagnosis of the disease. In addition,the measurement of blood serum, sugars, proteins, lipids will be veryhelpful in the diagnosis and treatment of the patients. The measurementof blood serum NT-proBNP is the sensitive index of HHD. |
