OBJECTIVE: By observing the different expression of urinary cystatin C andurinary α1-microglobulin in patients with chronic obstructive pulmonary disease patients,discussing sensitive biological indicators of early renal damage, contribute to the detectionof early renal dysfunction in patients with COPD,providing a theoretical basis for earlyclinical intervention and treatment.METHODS: Grouping: control group (healthy) of40cases,COPD group of48patients; the COPD group was divided into mild hypoxia group,moderate hypoxia group,severe hypoxia group and CO2retention group according to the results of blood gasanalysis, urinary cystatin C, urinary α1-microglobulin,urea nitrogen and creatinine weremeasured; In48cases,23cases of COPD patients performed spirometry, according tothe results of FEV1, the cases was divided intoFEV1≥50%group and FEV1<50%group,urinary cystatin C, urinary α1-microglobulin,were measured.RESULTS:1、Compared with the control group, the difference of urea nitrogen andcreatinine were not statistically significant in COPD group (P>0.05); but urinary cystatinC and urinary α1-MG levels in COPD group were significantly increased,the differenceswere statistically significant (P <0.01).2、In the three hypoxia groups which were accordingto the results of blood gas analysis in patients with COPD,(1)Compared with the controlgroup: In the mild hypoxia group,the difference of urine cystatin C and urinaryα1microglobulin levels were not statistically significant (P>0.05); In the moderate hypoxiagroup, urinary cystatin C levels were significantly increased, the differences werestatistically significant (P <0.01), but urinary α1-MG levels were not statisticallysignificant (P>0.05);In the severe hypoxia group, urinary cystatin C and urinary α1-MGlevels were significantly increased, the differences were statistically significant (P<0.05).3、Compared with hypoxia groups according to the results of blood gas analysis,the difference of urinary cystatin C and urinary α1-MG levels in CO2retention group weresignificantly increased,the differences were statistically significant (P <0.01).4、Comparedwith FEV1≥50%group, the difference of urinary cystatin C and urinary α1-MG levels inFEV1<50%group were significantly increased, the differences were statisticallysignificant (P <0.01).CONCLUSIONS:Both urinary Cys-C and urine α1-MG are biological markers ofearly renal tubular damage in COPD patients, and urinary Cys-C can be more sensitivethan the urine α1-MG to reflect early tubular damage cases in COPD patients.
