Analysis Of Causes And Countermeasures Of438Cases With Complicated Hand Injuries By Agricultural Machinery

Objective:In order to understand the characteristics of the complex agricultural machineryinjury of hand and the reasons, a total of438cases of hand complex agriculturalmachinery injury cases from China Japan Union Hospital of Jilin University wereretrospectively investigation over the period from January2012to November2014.This study is useful for improving the awareness of rural residents, reducing theincidence of agricultural machinery damage, providing suggestion andcountermeasures for agricultural machinery injury prevention and giving thetheoretical reference for implementation of the government and AIDS prevention.MATERIALS AND METHODS：This study adopts the method of retrospective analysis, a total of438cases ofhand complex agricultural machinery injury cases from Jilin province wereinvestigation over the period from January2012to November2014. the contents ofthis study mainly includes:(1) the patient general condition, for example gender andage;(2) the costs associated with the patient in the hospital, containing the total cost,operation cost, medicine cost and examination cost;(3) the situation of patient’s handinjury, containing the time of injury, injured part, multiple injuries, degree of tissuedamage, machinery factor of injury and treatment of injury.RESULTS：(1) In this study, there were438cases of hand complex agricultural machineryinjury patients, the youngest is only10years old, and the oldest is78year old,average age was43.5, which has the largest number of age groups with hand injuriesfor41-60,220cases, accounting for50.2%of the total patients, the group with ageless than20years old are19people, accounting for4.3%of the total. the group withage21-40contains124people, accounting for28.3%of the total,75people over the age of61, accounted for17.1%;(2) the male patients with308people, accounting for70.3%of the total. Women with130people, accounting for29.7%of the total, malepatients significantly more than women, and men’s and women’s total ratio is2.4:1;(3)from2012to2014, the hospitalization cost of patients with injury of hand, operationexpenses, expenses for medicine, treatment and examination are rising year by year,three years in the hospital total cost were16864.40±11988.41,17682.10±10164.20and19452.2112652.37yuan; Surgery were5140.19±4890.63,5432.07±5102.29and5707.74±5344.37yuan; Medicines were5506.02±4020.18,5984.39±4899.52and6012.04±5618.57yuan; Treatment were1440.51±1037.33,1556.74±1360.08and1643.29±1279.45yuan; Inspection fee were881.17±618.39,890.24±773.05and913.01±618.70yuan, respectively;（4）In spare time，there were47people injuries,accounting for10.7%of the total, the autumn harvest period up to268people withinjuries, accounting for61.2%of the total, the number of injuries for123springperiod, accounting for28.1%of the total patients;(5) The percentage of middle fingerinjury was the highest, at24.1%, followed by the ring finger (21.1%), index finger(19.4%), little finger (10.6%), the thumb (9.2%), the palm of hand wrist injury (9.0%),injury of forearm, at least6.6%;(6) hand multiple damage was found in261cases,accounting for59.6%of the total, single site injury in177cases, accounting for40.4%of the total; Different years, many parts damage cases gradually increased, in2012the number of cases was46cases, but in2014the number increased into139cases;(7) About hand complex tissue injury in patients with injury of agriculturalmachinery, the percentage of skin damage was highest, at28.2%, followed by thebone damage at21.0%, nerve injury (15.3%), tendon injuries (16.3%), vascular injuryaccounted for12.7%, and other injuries accounted for6.5%;(8) Corn thresher injurieswere in the first place, accounting for a total of193people(44.1%), followed by smallharvester, for122people, accounting for27.9%of the total, machine damage41people, accounting for9.4%, agricultural transport damage for54people,12.3%ofthe total,28people injured by other farm machinery, accounting for6.4%;(9)Because of the subjective factors such as fatigue, violating the operating rules,operation skilled, inattention, drinking, poor psychological status etc. in complicated hand agricultural injured87.2%, the number of agricultural injury and agriculturalusers in subjective factors lead to the increased year by year.Conclusion：(1) aged male in the rural41-60years old men were the high risk group for thecomplicated agricultural machinery injury in the countryside.(2) human subjective safety awareness is poor, illegal operation is the mostimportant cause of the hand of complex agricultural injury.(3) the impact of complex agricultural machinery injuries to the community andthe individual is enormous.
