Objective: To investigate the efficacy of Jinchuanggao and rhEGF(recombinant human epidermal growth factor) with establishing ratsmodel of diabetic skin ulcer, by observing the expression of VIM(vimentin), MMP-13(matrix metahoproteinases-13), DDR-1(discoidindomain receptor-1), FB (Fibroblasts), Col-Ⅰ (collagen Ⅰ), Col-Ⅲ(collagen Ⅲ) in diabetic rats skin ulcer, reveal the inner link along withthe change of time, and provide experimental basis for clinical treatment.Methods:1. The diabetes model were established and grouped:48diabetic skin ulcer rats were randomly divided into three groups, theexperimental group was Jinchuanggao group, control group wasepidermal growth factor group, and blank group. From the first day of thesuccessful modeling, every group would be changed dressing, fixed withmedical tape, until the ulcer healed and observed the treatment effect.2.Drew materials and stained: after1week and two weeks the woundtissue of rats, formaldehyde fixed, conventional paraffin embedding, HEstaining (hematoxylin-eosin staining; HE) to observe fibroblastmorphology, picric acid-Sirius red staining to observation changes ofcollagen Ⅰ, collagen Ⅲ area, immunohistochemical method (SABC)determination of MMP-13, DDR-1, the expression of VIM.3. Use IPP6.0(Image-Pro Plus6.0) Image analysis software tested the averageoptical density value of MMP-13, DDR-1, VIM (average opticaldensity, AOD).4. Analysis of Statistical data: the data denoted with themean±standard deviation (±s), the data was analyzed by used SPSS17.0statistical software, the same index was analyzed between twodifferent groups used single factor analysis of variance (one-wayANOVA), multiple comparisons used LSD test between groups, therewas a significant difference when P<0.05. Results:(1) The animalmodel of observations:1week and2weeks, observed the color of thewound, dry humidity, granulation tissue growth status, presence ofscarring, Wound healing95%healing as standard, recorded the woundhealing days. First results show that the three groups of ulcer woundhealing indifference, bright red color, seepage was obvious.1week and2weeks, the Jinchuanggao group compare with blank control group tissuefluid leakage decreased significantly, granulation tissue appeare early inthe blank group, and colour and lustre was ruddy, wound completelycovered by granulation tissue. Blank group are seeping, dark red color,granulation tissue grow later in the Jinchuanggao group, and unable tocompletely covere ulcer wound. The wound healing condition ofepidermal growth factor group between Jinchuanggao group and blankgroup. Ultimately heal, Jinchuanggao group heal fastest than other twogroups, and a little scar, similar to normal skin tissue, Blank group heale slower than Jinchuanggao group, scar bump in normal skin, is differentwith normal skin tissues. The epidermal growth factor group healingeffect between Jinchuanggao group and blank group, forms of scar tissuebetween Jinchuanggao group and blank group.(2) HE stained to observethe arrangement of fiber cells, granulation tissue growth situation,Jinchuanggao group of granulation tissue appear earlier than theepidermal growth factor group, and the arrangement of fibroblasts looklike "fingerprint", epidermal growth factor group compare with blankgroup appear early granulation tissue. The polarity of the fibroblasts ofblank group disappears and disordered arrangement of collagen fibers,granulation tissue appeared later.(3)2weeks, picric acid-Sirius redstained, Jinchuanggao group the area of collagen Ⅰ, collagen Ⅲcompare with other two groups, Jinchuanggao group the area of collagenⅢ is biggest, epidermal growth factor group bigger than blank group,blank group is the smallest. Blank group the area of Collagen Ⅰis thelargest, larger than epidermal growth factor group, Jinchuanggao group isthe smallest.(4) Immunohistochemistry tests every group the averageoptical density value of MMP-13, DDR-1, VIM, image analysis showsthat the average optical density value of3groups compare with, therewas significant difference(P <0.05).7day the average optical densityvalue of MMP-13, DDR-1, VIM compare with14days the averageoptical density value of MMP-13, DDR-1, VIM, there was significant difference(P <0.05).Conclusion:(1) Jinchuanggao and rhEGF in thetreatment of diabetes refractory ulcer, probably stimulating fibroblastproliferation and better coordinate the expression of MMP-13, DDR-1, VIM, then control the ratio of collagen Ⅰ and collagen Ⅲ, achievethe extracellular matrix remodeling.(2) Jinchuanggao in treatment ofdiabetic skin ulcer, the curative effect is better than the epidermal growthfactor, has a quick effect, little scar, etc.(3) In the ulcer healing process,MMP-13may as downstream product of DDR-1, feedback regulationcollagen Ⅰ and collagen Ⅲ in the extracellular matrix; Better controlexpression of gene VIM can effectively reduce the formation of scar.
