| OBJECTIVE:To explore the application value of splenic hemodynamic in posthepatitic cirrhosis splenomegaly and chronic hepatitis splenomegaly and hematopathy splenomegaly by color Doppler ultrasound.METHODS:The hemodynamic parameters of the splenic artery(SA),the splenic lobe artery (SA),the splenic vein behind pancreas(SV), the splenic lobe vein (SLV) and the portal vein (PV) of40cases with posthepatitic cirrhotic splenomegaly,31cases with chronic hepatitis splenomegaly,40cases with hematopathy splenomegaly and54normal controls were studied by color Doppler ultrasound.And the test results were analyzed by applying statistical methods.RESULTS:(1) In liver cirrhosis and hematopathy group, Vs of SA and SLA were higher than that in chronic hepatitis group(P<0.005).The blood flow velocity (Vs, Vd) of SA and SLA in liver cirrhosis group have no statistical difference compared with hematopathy group(P>0.05).(2) In hematopathy group, Vm of SV,SLV and PV was higher than those in other groups(P<0.005).(3) In liver cirrhosis group, Vm of PV was lower than those in other groups(P<0.05).The Vm of PV in chronic hepatitis group has no statistical difference compared with normal controls.(4) In liver cirrhosis group, the inner diameter of PV was bigger than those in hematopathy group and chronic hepatitis group and normal controls(P<0.001).(5) In liver cirrhosis group, the average blood flow of in per minute of SV was higher than those in hematopathy group and chronic hepatitis group and normal controls(P<0.05).The average blood flow of in per minute of SV,SLV and PV of normal controls were lower than those in other groups(P<0.001).CONCLUSION:It’s helpful to distinguish between posthepatitic cirrhosis splenomegaly and chronic hepatitis splenomegaly and hematopathy splenomegaly to detect the hemodynamic parameters by color Doppler ultrasonography. It is beneficial to guide the clinical to intervention treatment,and it has the clinical application value. The Vm of PV is the better indicator to distinguish between posthepatitic cirrhosis splenomegaly and chronic hepatitis splenomegaly and hematopathy splenomegaly. |
