Objective:To explorer the correlation between blood uric acid level and the severity of clinical and histopathological parameters in the IgA nephropathy patients with Uyghur nationality. Methods: ninety-nine Uyghur IgA nephropathy patients diagnosed by renal biopsy in people’s hospital of uygur autonomous in xinjiang from2012.1to2013.6were recruited randomly in this study,Which were classified into two groups according to the blood uric acid level:IgA nephropathy with hyperuricemia (group A) and control group (group B).The clinical date including genderã€ageã€body mass indexã€hypertension blood pressureã€uric acidã€renal functionã€renal funtionã€blood urea nitrogenã€cholesterolã€triglycerides〠high-density lipoprotein cholesterolã€low-density lipoprotein cholesterolã€uric acidã€urine creatinine and24h urine protein quantitative) and histopathological date were collected.The differences of the clinical and pathological features were studied retrospectively between the group. The influence factors of renal pathological damage of IgA nephropathy were analyzed by linear correlation; The risk factors of IgA nephropathy with high uric acid hematic were analyzed by multivariate linear regression.ResultS:(1)The incidence of high blood uric acid in Uighur IgA nephropathy patients was17.17%;(2) Compared with control group, body mass index (BMI), systolic blood pressure, blood glucose, serum creatinine, urea nitrogen, total cholesterol, triglyceride, low density lipoprotein cholesterol, lipoprotein and24hours urinary protein were significantly higher and glomerular filtration rate was lower in group A.(3) Pathology Lee’s classification is higher and glomerular damage is heavier in group A, When blood uric acid level is higher than360mmol/L, the incidence of renal interstitial fibrosis rises significantly.(4) Spearman correlation analysis results show that blood uric acid, creatinine and24h urinary protein positively correlated with degree of glomerular injury, serum creatinine is strongest related to glomerular sclerosis and pathological changes of blood vessels, blood uric acid strongest correlated with formation of crescent.(5) Multiple linear regression analysis shows that the BMI, male, hemal wall lesion, triglycerides, blood creatinine were independent risk factors of high uric acid hematic disease in IgA nephropathy patients with Uyghur nationality.Conclusion:The clinical and pathological damage degree is more serious in Uygur IgA nephropathy patients with hyperuricemia than with normal blood uric acid.High uric acid may accelerate the progression of Uighur IgA nephropathy. |
