Objective:To study the value of fiberoptic bronchoscopy on the early diagnosis of heat stroke patients with respiratory infection,thereby improving the early diagnosis of heat stroke patients with respiratory infection.Method:16 patients with heat stroke admitted to the ICU of our hospital were enrolled from May 2012 to July 2014 and randomly divided into conventional group and bronchoscopy group. The airways of all patients were opened.11 patients were treated with oral trachea cannula,5 patients with nose oral trachea cannula. Sputum samples were collected at 8am of everyday from the first days after admitted the ICU. The first conventional group was treated with suction tube.The second group was the bronchofiberscope group, patients lie on the back, using the multi-function ecg monitoring, continuous monitoring of patients’ heart rate, blood pressure, breathing rate and blood oxygen saturation. After bronchoscopy across the glottis, secretions was collected in the most learned of the secretions, the specimens were sent to laboratory for bacterial culture and susceptibility testing.Result (1) First,bacteria was not cultivated in 16 cases of heat stroke patients within 24 hours after admiting the ICU, but different bacteria was cultivated within 48 hours,accounting for 6% of the conventional group,12% of the bronchoscopy group, which was significantly higher than the conventional group.(2) Multiple drug-resistant bacteria were detected in the bronchoscopy group after five day admitting the ICU, the sevend day of the conventional group.44 strains of multi drug resistant bacterias were cultured in the the specimen within 7days, the positive rate was 78.57%, of which regular group 16, the positive rate of 36.36%; bronchoscopy group 28, the positive rate of 63.64%, the significant difference(P<0.05) between the two groups was observed.31 strains of Gram-negative bacteria (70.46%) were cultured,11 strains in conventional group (25%),20 in bronchoscopy group(45.45%), the significant difference(P<0.05) between the two groups was observed; 15 strains of acinetobacter baumannii mainly copper (34.09%) were cultured, 7 strains in conventional group (15.9%),8 in bronchoscopy group (18.18%), there were no significant difference between the two groups (P>0.05);13 strains of green Pseudomonas (29.55%) were cultured,7 strains in the conventional group (15.91%),6 in the bronchoscopy group (13.64%),there were no significant difference between the two groups (P>0.05);10 strains of Gram-positive bacteria (22.73%) were cultured,3 strains in the conventional group (6.8%),7 in the bronchoscopy group (15.91%), the significant difference between the two groups (P<0.05) was observed be,6 strains Gram-positive bacteria were mainly Staphylococcus aureus (13.64%) detected in the bronchoscopy group, the other were Streptococcus pneumoniae and Staphylococcus epidermidis.Conclusion:1.The diagnose of fiberoptic bronchoscopy for heat stroke patients complicated with early respiratory infection is simple,safe and more accuracy.2.The detection rate of pathogens and multi-drug resistant pathogens by bronchoscopy is more higher than the conventional suction... |
