Research background and purpose:By comparing the effects of alfentanil and sufentanil combined with propofol on patients’ coughing response,vital signs,adverse reactions,recovery time from anesthesia,operator and patient satisfaction during fiberoptic bronchoscopy Feasibility and safety of alfentanil combined with propofol in fiberoptic bronchoscopy.Materials and Methods:This study is a randomized,controlled,double-blind clinical study.200 cases of patients undergoing fiberoptic bronchoscopy who were admitted to the First People’s Hospital of Neijiang City from September 1,2021 to November 30,2021 were selected,and random number table method was used.200 patients were randomly divided into the experimental group alfentanil group(N=100)and the control group sufentanil group(N=100).Intravenous anesthesia was used in both groups.After the establishment of intravenous access,the alfentanil group was given intravenous injection of alfentanil hydrochloride 10ug/kg,and then intravenous injection of propofol 1.5-3.0mg/kg.The sufentanil group was given intravenous injection of sufentanil citrate 0.2ug/kg,and then immediately given intravenous injection of propofol 1.5-3.0mg/kg.According to the patient’s condition,propofol 3ml/time was added during the examination.Record the patient’s heart rate(times/min),blood pressure(mm Hg),mean arterial pressure(mm Hg),blood oxygen saturation(%)and respiration 3 minutes before the start of anesthesia,when the fiberoptic bronchoscope passes through the patient’s glottis,and immediately after the examination is completed.Frequency(times/min);lowest oxygen saturation during and after operation(%);occurrence of coughing response in patients during anesthesia induction;degree of body movement response,trachea and bronchospasm in patients during operation;Cough,bradycardia,whether to use assisted breathing;the total amount of propofol used in patients after surgery,the total time of examination,the time required for the patient to wake up,the time for recovery of orientation,the time for resuscitation,and the time for returning to regular breathing;intraoperative anesthesia Outcome ratings,operator satisfaction,patient satisfaction,and patient willingness to be re-examined.Result:1.Compared with the sufentanil group,when the fiberoptic bronchoscope passed through the glottis,the alfentanil group had a lower incidence of coughing reaction,body movement response,and bronchospasm,and the difference was statistically significant(P< 0.05);2.Compared with the sufentanil group,the incidence of coughing reaction in the alfentanil group was lower than that in the sufentanil group during anesthesia induction,and the difference was statistically significant(P<0.05);3.Compared with the sufentanil group,the heart rate of patients in the alfentanil group was lower during operation,and the difference was statistically significant(P<0.05);The saturation was higher than that of sufentanil group,and the difference was statistically significant(P<0.05);4.Compared with the sufentanil group,the postoperative alfentanil group used less propofol and the time required for examination,waking up,orientation recovery,and recovery were shorter,and the difference was statistically significant(P<0.05);however,there was no significant difference in the recovery time of regular breathing between the two groups after operation(P>0.05).Conclusion:1.Alfentanil combined with propofol can reduce the incidence of coughing response,body movement response,and bronchospasm during the induction period of patients and when the fiberoptic bronchoscope passes through the glottis;2.Alfentanil combined with propofol can provide better hemodynamics in fiberoptic bronchoscopy;3.Alfentanil combined with propofol in fiberoptic bronchoscopy can reduce the dosage of propofol,shorten the time required for examination,and shorten the recovery time related to anesthesia.
