Objective:Therapy of "tonifying qi and activating yang, harmonizing ying for migratory arthralgia" is proposed in Chapter "Blood-arthralgia Disease" of Golden Chamber. Since Zhongjing, numerous medical experts of late generations have discussed about the therapy of "guide yang-qi by needle" and its main representative prescription huangqi-guizhi-wuwu decoction, but systematic summary of its relevant theory and extensive application has not been available yet, such as differences and connections between blood-arthralgia and arthralgia, related qi and blood, and physiology and pathology of Ying-wei. Many medical experts’discussions emphasized on the pathogenesis related to "guide yang-qi by needle", but little on what channel should be sought and how to manipulate needles. This paper explores the issues raised above in order to play a positive significance for the exploration of the syndrome differentiating law and research idea of "tonifying qi and activating yang, harmonizing ying for migratory arthralgia", as well as clinical guidance in the future.Methods:According to the "blood-arthralgia disease"proposed in the Golden Chamber, this paper sorts the ancient Chinese medicine experts’ understanding of blood-arthralgia disease’s pathogenesis, traced to the pre-Qin era of the Canon of Internal Medicine, down to the Ming and Qing dynasties. Elaborate the theoretical basis of "tonifying qi and activating yang, harmonizing ying for migratory arthralgia", clarify the physiology and pathology of qi, blood, ying and wei, and explain differences and connections between blood-arthralgia and arthralgia. Explore approaches to seek channels and manipulate needles proposed in the Golden Chamber, and then make an analysis in combination of the formula-syndrome and drug-syndrome of Chinese classical prescription—huangqi-guizhi-wuwu decoction in Article Blood-arthralgia Disease of the Golden Chamber. Collect and sort numerous medicine experts’ experiences in clinical application based on this therapy, then take pathogenesis as the key to expand its application in clinical practice.Results:By sorting the ancient Chinese medicine experts’understanding of blood-arthralgia, this paper draws the following conclusion: blood-arthralgia’s pathogenesis is mainly caused by deficiency of qi and blood and disharmony between ying and wei internally and infection of wind pathogen externally. Meanwhile, both blood-arthralgia and arthralgia have their own connotation and contact, while the physiology and pathology characteristics of qi, blood, ying and wei are also their theoretical basis, so "guide yang-qi by needle" should be shallow needling on the he point of yang channel. Taking pathogenesis as the key in the clinical application of huangqi-guizhi-wuwu decoction is not only limited in blood-arthralgia disease, but can also be applied to a lot of miscellaneous diseases.Conclusion:The application of "tonifying qi and activating yang, harmonizing ying for migratory arthralgia" takes pathogenesis of deficiency of qi and blood, disharmony between ying and wei and blockage of yang blood as the key. The patients with a superficial disease location should apply superficial acupuncture into the he point of yang channel; for severe patients, huangqi-guizhi-wuwu decoction is used as the basic prescription to harmonize ying and wei and tonify qi and blood, achieving the effects of tonifying qi and activating yang, as well as harmonizing ying for migratory arthralgia. With the changing of the syndrome, other therapies can be supplemented when this therapy has been used in clinical practice. This is not only limited in blood-arthralgia disease, but also has a guiding significance for many miscellaneous diseases, such as heart and encephalopathy syndrome, spleen and stomach disorders, disease of qi-blood-body fluid, gynecology and dermatology diseases, etc. |
