Objective:To compare the clinical effect of surgery before neoadjuvant chemotherapy in the treatment of different histological types of locally advanced cervical cancer. Methods:A retrospective analysis of 110 cases of patients with locally advanced cervical cancer and follow-up, divided into squamous cell carcinoma(SCC) group (67 cases) and non-SCC group (43 cases), two groups were given intravenous chemotherapy of TP regimen, chemotherapy after operation treatment. Analysis of the effect of chemotherapy were compared between the two groups and the recent effects on operation, chemotherapy responders were compared between the two groups of patients with chemotherapy ineffective postoperative pathology and survival rate in 5 years. Results: the response rate was 75.45%(83/110), the group of SCC is 83.58%(56/67), and non-SCC is 62.79%(27/43), there was significant difference between two groups (χ2=6.113, P<0.05). We compared two groups of patients with tumor size before chemotherapy and after chemotherapy, no statistically significant results of two groups of patients before chemotherapy tumor size difference (P>0.05), SCC after chemotherapy the tumor diameter size was significantly smaller than non-SCC group, the difference was statistically significant (P<0.05). In cervical cancer neoadjuvant chemotherapy is effective in postoperative patients with lymph node positive rate was21.43%(12/56), chemotherapy is ineffective in postoperative patients with lymph node positive rate was 72.73%(8/11) Comparison of the two groups, the difference was statistically significant (P<0.05); SCC chemotherapy in patients with non-effective postoperative lymph node positive rate was 18.52%(5/27), ineffective chemotherapy patients postoperative lymph node positive rate was 81.25%(13/16), the two groups, the difference was statistically significant (P<0.05).Conclusion:Cervical cancer preoperative NACT can make local tumor volume reduction, is conducive to the operation, and the patients can be tolerated, But the recent curative effect of neoadjuvant chemotherapy for cervical squamous cell carcinoma than in SCC of the good. SCC and non-SCC preoperative chemotherapy of TP regimen had no significant difference between the effects of recent operation. Both SCC or non-SCC, pelvic lymph node metastasis rate of chemotherapy responders than chemotherapy nonresponders is low, and the 5 year survival rate of effective chemotherapy are higher than chemotherapy is ineffective, which may improve the prognosis.
